摘　　要：目的探讨经肋间单孔胸腔镜(IUVATS)肺叶切除术联合消癌平治疗非小细胞肺癌(NSCLC)的临床疗效,以及对患者微小RNA210(miR-210)和微小RNA101(miR-101)表达水平的影响。方法选取医院2020年4月至2021年4月收治的NSCLC患者118例,按随机数字表法分为对照组和观察组,各59例。对照组患者以IUVATS肺叶切除术治疗,并辅以PE(顺铂+依托泊苷)化疗方案;观察组患者在对照组治疗基础上加用消癌平注射液。以21 d为1个化疗周期,两组均连续治疗4个周期。结果观察组患者的客观缓解率(ORR)为77.97%,疾病控制率(DCR)为88.14%,显著高于对照组的61.02%和71.19%(P<0.05)。治疗后,观察组患者的肿瘤标志物[糖类抗原199(CA199)、糖类抗原153(CA153)、人细胞角蛋白21-1片段(CYFRA21-1)]均显著低于对照组(P<0.05);T淋巴细胞亚群CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+)均显著高于对照组,CD_(8)^(+)显著低于对照组(P<0.05);miR-210和miR-101水平均显著低于对照组(P<0.05)。治疗期间,观察组和对照组患者的不良反应发生率相当(18.64%比13.56%,P>0.05)。结论IUVATS肺叶切除术联合消癌平治疗NSCLC的临床疗效良好,能降低患者的肿瘤标志物水平,增强免疫力,抑制miR-210和miR-101的表达,且治疗安全性较好。Objective To investigate the clinical efficacy of intercostal uniportal video-assisted thoracoscopic surgery(IUVATS)lobectomy combined with Xiaoaiping in the treatment of non-small cell lung cancer(NSCLC),and its effects on the expression levels of microRNA210(miR-210)and microRNA101(miR-101)in patients.MethodsA total of 118 patients with NSCLC admitted to the hospital from April 2020 to April 2021 were selected and divided into the control group and the observation group by the random number table method,with 59 cases in each group.The patients in the control group were given the IUVATS lobectomy and PE(cisplatin+etoposide)chemotherapy regimen,on this basis,the patients in the observation group were treated with Xiaoaiping Injection.Both groups were treated continuously for four cycles with 21 d as a chemotherapy cycle.ResultsThe objective remission rate(ORR)in the observation group was 77.97%,the disease control rate(DCR)in the observation group was88.14%,which were significantly higher than 61.02%and 71.19%in the control group,respectively(P<0.05).After treatment,the levels of tumor markers[carbohydrate antigen 199(CA199),carbohydrate antigen 153(CA153)and human cytokeratin fragment antigen 21-1(CYFRA21-1)]in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the T lymphocyte subgroups including CD_(3)^(+),CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)in the observation group were significantly higher than those in the control group,and the CD_(8)^(+)in the observation group was significantly lower than that in the control group(P<0.05).After treatment,the levels of miR-210 and miR-101 in the observation group were significantly lower than those in the control group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group was similar to that in the control group(18.64%vs.13.56%,P>0.05).ConclusionIUVATS lobectomy combined with Xiaoaiping is effective and safe in the treatment of NSCLC,which can reduce the tumor marker levels,improve immuni
