颧骨降低颧弓内推术联合中面部除皱术的临床效果观察

摘　　要：目的 观察颧骨降低颧弓内推术联合中面部除皱术在改善面部轮廓和预防中面部松垂中的临床效果。方法 自2016年1月至2020年1月,重庆当代整形美容医院对52例患者行颧骨降低颧弓内推术联合中面部除皱术。评估患者术后中面部缩窄情况,分别采用皱纹严重程度评分表和面部整体美容效果改善分级表对术后鼻唇沟皱纹严重程度和面部整体美容效果进行评价。结果 52例患者术后未出现面神经损伤、感染、秃发、皮肤坏死等并发症。术后随访6~12个月,患者对术后面部外形的改善效果满意,耳前-颞区除皱术切口瘢痕不明显。术后6个月中面部宽度较术前缩窄(P<0.05);手术前后鼻唇沟皱纹严重程度评分差异有统计学意义(P<0.05);面部整体美容效果均有不同程度改善,未出现无效或恶化病例。结论 颧骨降低颧弓内推术联合中面部除皱术在改善中面部轮廓的同时,可以有效地预防中面部松垂的发生,是值得推广的联合手术方式。Objective To observe the clinical effect of zygoma and zygomatic arch osteotomy combined with mid-face lift surgery in improving facial contour and preventing mid-face sagging. Methods From January 2016 to January 2020, 52 patients who were subjected to zygoma and zygomatic arch osteotomy combined with mid-face lift surgeries were selected. The following items before and after the surgery were evaluated and compared including mid-face narrowing, wrinkle of nasolabial groove(using the wrinkle severity rating scale,WSRS), and global aesthetic improvement(using the global aesthetic improvement scale, GAIS). Results After 6 to 12 months of follow-up, all 52 patients were satisfied with the operation improvement. The preauricular and temporal incision for the mid-face lift surgery presented no obvious scars. Complications included facial nerve injury, infection, baldness, and cutaneous necrosis were not reported. Six months post-operation, the mid-face width was significantly reduced(P<0.05). Nasolabial groove were significantly improved(P<0.05).Global aesthetic effect also presented improvement in different degrees, with no unaltered or worsened case. Conclusion The combined surgery method of zygoma and zygomatic arch osteotomy with mid-face lifting could effectively improve facial contouring and prevent mid-face sagging simultaneously. This is a surgical method worthy of promotion.
