老年男性2型糖尿病患者尿酸排泄分数与骨质疏松症的相关性

摘　　要：目的探讨老年男性2型糖尿病患者尿酸排泄分数(FEUA)与骨质疏松症(OP)的相关性。方法选取2019年7月至2021年7月老年男性2型糖尿病患者219例,根据骨密度(BMD)将入组患者分为骨量正常组(77例)、骨量减少组(72例)、骨质疏松组(70例)。分析老年男性2型糖尿病患者FEUA与OP的相关性。结果三组患者HbA1c、FPG、UA、Cr、TG、Ca、25-(OH)D、FN BMD、L_(1~4)BMD差异有统计学意义(P<0.05)。随着FEUA的增加,老年男性2型糖尿病患者L_(1~4)、股骨颈、Wards三角、大粗隆的BMD呈下降趋势(P<0.05)。患者FEUA水平由低到高依次为骨量正常、骨量减少组和骨质疏松组(P<0.05)。FEUA水平与L_(1~4)、股骨颈、Wards三角、大粗隆的BDM均呈负相关(P<0.05)。HbA1c、UA、Cr、TG、25-(OH)D、FEUA是骨密度的影响因素(P<0.05)。ROC曲线分析提示FEUA对OP具有一定的预测价值(曲线下面积为0.73,P=0.03),Youden指数最大对应的FEUA为12.27%。结论老年2型糖尿病男性患者FEUA水平与OP负相关,FEUA>12.27%对OP具有一定的预测价值,其机制可能与FEUA反应机体的氧化应激状态有关,后续有待进一步研究证实。Objective To explore the correlation between uric acid excretion fraction(FEUA)and osteoporosis(OP)in elderly male patients with type 2 diabetes mellitus.Methods 219 elderly male patients with type 2 diabetes were selected from the Department of Endocrinology,Hangzhou Traditional Chinese Medicine Hospital from July 2019 to July 2021.According to bone mineral density(BMD),the patients were divided into the normal bone mass group(77 cases),the osteopenia group(72 cases)and the osteoporosis group(70 cases).The correlation between FEUA and OP in elderly male patients with type 2 diabetes was analyzed.Results There were significant differences in HbA1c,FPG,UA,Cr,TG,Ca,25-(OH)D,FN BMD,L_(1~4)BMD among the three groups(P<0.05).With the increase of FEUA,BMD of L_(1~4),femoral neck,Wards triangle and greater trochanter in elderly male patients with type 2 diabetes significantly decreased(P<0.05).The levels of FEUA in the elderly patients with type 2 diabetes mellitus from low to high were normal osteoporosis,osteopenia and osteoporosis,with statistical significance(P<0.05).FEUA levels were negatively correlated with L_(1~4),femoral neck,Wards triangle and greater trochanteric BDM(P<0.05).HbA1c,UA,Cr,TG,25-(OH)D and FEUA were the influencing factors of bone mineral density(P<0.05).ROC curve analysis showed that FEUA had a certain predictive value for OP(the area under the curve was 0.73,P=0.03),and the largest FEUA corresponding to Youden index was 12.27%.Conclusion FEUA level is negatively correlated with OP in elderly male patients with type 2 diabetes mellitus,and FEUA>12.27%has a certain predictive value for OP.Its mechanism may be the reaction of FEUA on oxidative stress state of the body,which needs further study.
