摘 要:目的探讨肱骨头不同部位囊变与肩袖损伤程度、肩峰指数、肩峰角、最短肩峰间距的相关性。方法回顾性分析348例肩关节磁共振资料。按肱骨头囊变部位分四组:小结节组、大结节前组、大结节后组,解剖颈裸区组。分析各囊变组与无囊变组肩袖损伤和肩峰指数、肩峰角、最短肩峰间距的差异。结果348例中肱骨头囊变223例。小结节组肩袖损伤程度大于无囊变组,最短肩肱间距小于无囊变组;大结节前组肩袖损伤程度大于无囊变组;大结节后组肩袖损伤程度、肩峰角、肩峰指数大于无囊变组;差异均有统计学意义(P<0.05)。结论肩袖损伤与肱骨头囊变、肩峰下撞击之间存在一定的联系,肱骨头囊变与冈上肌腱损伤密切相关,肩峰下撞击会促进肱骨头囊变的发生。Objective To investigate the correlation between cystic changes in different parts of the humeral head and rotator cuff injury,acromion index,acromion angle and shortest acromion distance.Methods The MRI data of 348 cases of shoulder joint were analyzed retrospectively.According to the location of cystic change of the humeral head,they were divided into four groups:the small tubercle of humerus group,the anterior tubercle of humeral head group,the posterior tubercle of humeral head group and the bare area of humeral anatomical neck group.The differences of rotator cuff injury,acromion index,acromion angle and shortest acromion distance between the cystic group and the non-cystic group were analyzed.Results 223 cases(64%)of 348 cases had cystic degeneration of humeral head.The degree of rotator cuff injury in the small nodule group was greater than that in the non-cystic group,and the shortest shoulder brachial distance was smaller than that in the non-cystic group.The degree of rotator cuff injury in the anterior micronodule group was greater than that in the non-cystic group.The degree of rotator cuff injury,acromion angle and acromion index in the posterior micronodule group were greater than those in the non-cystic group.The above differences were statistically significant(P<0.05).There was no significant difference in other indicators(P>0.05).Conclusion There is a certain relationship between rotator cuff injury and humeral head cystic change and subacromial impact.Humeral head cystic change is closely related to supraspinatus tendon injury.Subacromial impact will promote the occurrence of humeral head cystic change.
