摘  要:目的:探讨新发房颤(NOAF)对念珠菌血症患者近期及远期预后的影响。方法:收集重症监护病房(ICU)诊断为念珠菌血症的患者92例,根据是否存在心房颤动(AF)将患者分为:NOAF组(26例)、既往房颤组(7例)、无房颤组(59例)。比较3组患者间的一般资料、实验室指标、院内死亡率及出院后3年内生存情况。结果:NOAF组患者机械通气时间及院内死亡率明显高于无房颤组(P均<0.05);且Kaplan-Meier生存分析显示NOAF对念珠菌血症患者的生存率具有明显影响,即NOAF组患者的远期生存率明显低于无房颤组(P<0.05)。结论:NOAF在念珠菌血症患者中发病率高,且明显升高患者的近期及远期死亡率。Objective:To analyze the effect of new-onset atrial fibrillation on the short-term and long-term mortality in patients with candidemia.Methods:The clinical data of 92 patients diagnosed with candidemia and admitted into the intensive care unit were retrospectively analyzed.Based on the pathological condition of atrial fibrillation,the patients were divided into new-onset atrial fibrillation group(NOAF group,n=26),previous fibrillation group(n=7)and no atrial fibrillation group(n=59).Baseline characteristics,laboratory findings,in-hospital mortality and 3-year survival were compared.Results:The duration of mechanical ventilation and in-hospital mortality in NOAF group were significantly increased as compared with those in no atrial fibrillation group(all P<0.05).Kaplan-Meier analysis showed that NOAF significantly influenced the long-term survival of patients with candidemia,and the long-term survival rate in NOAF group was significantly lower than that in no atrial fibrillation group(P<0.05).Conclusion:The incidence of NOAF is high in patients with candidemia,which significantly increases the short-term and long-term mortality.
