中医定向透药治疗亚急性甲状腺炎急性期的临床研究

摘　　要：目的观察中医定向透药治疗亚急性甲状腺急性期的临床疗效。方法将45例亚急性甲状腺炎患者随机分为对照组、金黄散组与消瘿散组各15例。对照组予口服泼尼松片,金黄散组在对照组基础上加用金黄散中医定向透药治疗,消瘿散组在对照组基础上加用消瘿散中医定向透药治疗,共治疗2周。观察各组患者治疗前后疼痛视觉模拟量表(VAS)评分、甲状腺功能、红细胞沉降率(ESR)、C反应蛋白(CRP)、甲状腺体积、中医证候积分等指标。结果各组患者治疗后VAS评分、中医证候积分、ESR、CRP均较治疗前降低,金黄散组与消瘿散组降低更为明显(P<0.05),且消瘿散组止痛效果优于金黄散组(P<0.05);各组患者治疗后甲状腺功能、体积均较治疗前改善,金黄散组与消瘿散组改善更为明显(P<0.05)。结论中医定向透药疗法可有效、快速地缓解症状,减轻炎症反应,恢复甲状腺功能,疗效优于单纯激素治疗,安全可靠,并且消瘿散的止痛效果优于金黄散。Objective:To observe the clinical efficacy of TCM directed permeation in the treatment of subacute thyroid acute phase.Methods:A total of 45 patients with subacute thyroiditis were randomly divided into three groups,15 cases in each group.The control group was given oral prednisone tablets,the Jinhuang ointment group was treated with TCM directed permeation with Jinhuang ointment on the basis of the control group,the Xiaoying ointment group was treated with TCM directed permeation with Xiaoying ointment on the basis of the control group.All groups were treated for 2 weeks.The visual analogue scale(VAS),thyroid function,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),thyroid volume,TCM syndrome score of the three groups were observed.Results:The VAS score,TCM syndrome score,ESR and CRP of the three groups decreased significantly after treatment,and the Jinhuang ointment group and the Xiaoying ointment group decreased more significantly(P<0.05),and the analgesic effect of the Xiaoying ointment group was better than that of the Jinhuang group(P<0.05);the thyroid function and volume of the three groups improved after treatment compared with those before treatment,and the Jinhuang group and the Xiaoying ointment group improved more obviously(P<0.05).Conclusion:TCM directed permeation therapy can effectively and quickly relieve the acute symptoms of subacute thyroiditis,and the efficacy is better than glucocorticoid therapy.The analgesic effect of Xiaoying ointment is better than that of Jinhuang ointment.
