从“热毒”论治重症肺炎探析

摘　　要：重症肺炎属中医学“风温肺热病”范畴,是因风热病邪犯肺,热壅肺气,肺失清肃而致发热、咳嗽、胸痛为主症的急性肺部炎性病变。因“热毒”贯穿本病起病、病极、恢复的整个过程,故根据“热毒”在不同时期的致病特点,将本病分为热毒犯肺期、热(毒)入心包期、(热)毒损肺络期,并针对每期不同病机,分别给予“截断扭转”“清心开窍,救逆固脱”“益气养阴,肺胃同治”的不同治法,为其临床治疗提供了新的思路。Severe pneumonia belongs to the category of“wind-warm disease with lung heat”in Chinese medicine.It is an acute inflammatory lung disease characterized by fever,cough and chest pain due to the wind heat evil invading the lung,heat obstructing the lung qi,and lung failing to purge.Because“heat toxin”runs through the whole process from the onset,extreme disease,to the recovery of the disease,according to the pathogenic characteristics of“heat toxin”in different periods,the disease is divided into three stages:the stage of heat toxin invading the lung,the stage of heat(toxin)entering the pericardium,and the stage of(heat)toxin damaging the lung collaterals,and different treatments are given for“heat toxin”,such as“cutting off and reversing”,“clearing the heart and opening the orifices,saving the enemy and removing the solid”,and“supplementing qi and nourishing yin,and treating the lung and stomach together”,which provide new ideas for its treatment.
