筋膜松解针法结合常规针刺治疗主观性耳鸣临床研究

摘　　要：目的:观察筋膜松解针法结合常规针刺治疗主观性耳鸣的临床疗效。方法:选取2021年5月—2021年11月在黑龙江中医药大学附属第一医院门诊耳鼻咽喉科、黑龙江省医院全科医疗科及老年神经内科就诊的主观性耳鸣患者61例为研究对象,随机分成观察组与对照组。观察组采用筋膜松解针法结合常规针刺,对照组采用常规针刺,1次/d,5 d为1个疗程,连续治疗两个疗程。观察治疗前、后两组患者耳鸣残疾评估量表(THI)、耳鸣评价量表(TEQ)、焦虑量表(SAS)评分、视觉模拟评分法(VAS)评分及TCD基底动脉平均血流速度峰值,并比较临床疗效。结果:治疗后两组患者THI、TEQ、SAS和VAS评分与治疗前相比均显著降低,差异有统计学意义(P<0.05);治疗后,观察组患者THI、TEQ、SAS和VAS评分与对照组相比均显著较低,差异有统计学意义(P<0.05);治疗后,观察组TCD基底动脉平均血流速度峰值较对照组明显增高,差异有统计学意义(P<0.05)。治疗后,观察组愈显率80%(24/30),总有效率96.67%(29/30);对照组愈显率30%(9/30),总有效率76.67%(23/30),观察组临床疗效明显优于对照组,差异有统计学意义(P<0.05)。结论:应用肌筋膜触发点理论,采用筋膜松解针法结合常规针刺对主观性耳鸣具有良好的临床疗效。Objective:To observe the clinical effect of fascia decompression acupuncture combined with conventional acupuncture in treatment of subjective tinnitus.Methods:61 patients with subjective tinnitus were randomly divided into the observation group and the control group.The observation group was treated with fascia decompression acupuncture combined with conventional acupuncture,whereas the control group was only treated with conventional acupuncture,1 time/d,5 d was taken as one course of treatment,both groups were treated for two courses.The scores of THI,TEQ,SAS and VAS,as well as TCD basilar artery mean blood flow velocity peak were observed before and after the treatment in the two groups.The clinical efficacy was compared between the two groups.Results:The scores of THI,TEQ,SAS and VAS were significantly decreased after the treatment compared with those before the treatment in the two groups(P<0.05);the scores of THI,TEQ,SAS and VAS were significantly lower in the observation group than those in the control group after the treatment(P<0.05).The peak average blood flow velocity of basilar artery in TCD was significantly higher in the observation group than that in the control group after the treatment(P<0.05).The remarkable curative rate was 80%(24/30)and the total effective rate was 96.67%(29/30)in the observation group,which were significantly higher than 30%(9/30)and 76.67%(23/30)respectively in the control group(P<0.05).Conclusion:Fascia decompression acupuncture combined with conventional acupuncture has a good clinical effect on subjective tinnitus,based on the theory of myofascial trigger points.
