基于“分肉”理论探讨超声下刃针筋膜松解治疗颈性头痛的临床疗效及机制

摘　　要：目的:从“分肉”理论探讨超声下刃针筋膜松解治疗颈性头痛的临床疗效及机制。方法:将符合诊断标准的57例患者随机分为刃针组28例与对照组29例。刃针组采取超声下刃针筋膜松解治疗,对照组常规针刺治疗,观察两组治疗前、治疗1个疗程后、治疗2个疗程后头痛强度积分、头痛频率积分和头痛持续时间积分情况,比较治疗2个疗程后两组临床疗效。结果:治疗1个疗程后、治疗2个疗程后,头痛强度积分、头痛频率积分与头痛持续时间积分刃针组明显优于对照组,差异有统计学意义(P<0.05);刃针组于总疗效方面优于对照组,差异有统计学意义(P<0.01)。结论:超声下刃针筋膜松解治疗颈性头痛的临床疗效较好,机制可能与从筋论治、对“分肉”之间的筋膜松解有关。Objective:To explore clinical efficacy and mechanism of blade needle fascial release under ultrasound in treatment of cervicogenic headache(CEH)based on theory of flesh segmentation.Methods:57 patients who met the diagnostic criteria were randomly divided into the blade needle group(n=28)and the control group(n=29).The blade needle group was treated with ultrasound-assisted blade needle fascial release,and the control group was treated with conventional acupuncture.The scores of headache intensity,headache frequency,and headache duration were observed in the two groups before the treatment,after 1 course of the treatment and after 2 courses of the treatment.The clinical efficacy was compared between the two groups after 2 courses of the treatment.Results:After 1 course of the treatment and 2 courses of the treatment,the scores of headache intensity,headache frequency and headache duration in the blade needle group were significantly better than those in the control group(P<0.05).The total clinical efficacy was significantly higher in the blade needle group than that in the control group(P<0.01).Conclusion:The clinical efficacy of blade needle fascial release under ultrasound is significant in treatment of CEH,and the mechanism may be related to the fascia release between flesh segmentation based on meridian sinew.
