摘 要:目的利用UPLC-MS(超高液相-质谱联用)技术,探索肺脾气虚型慢性鼻-鼻窦炎不伴鼻息肉患者(CRSsNP)与空白组的鼻粘膜中内源性代谢产物的差异,以及口服“益气通窍饮”后内源性代谢物的变化情况,从而揭示肺脾气虚型CRSsNP的辨证物质基础和治疗靶点。方法选取2019年4月~2021年10月,肺脾气虚型CRSsNP 32例(治疗组),空白对照组20例,采用UPLC-MS检测两组鼻粘膜中内源性代谢产物,对潜在标记物进行筛选以及代谢途径进行分析。结果与空白组相比,发现治疗组中显著性差异代谢物20个,涉及的代谢通路主要有5条,口服“益气通窍饮”八周后差异性代谢产物均显示回调。结论(1)基于UPLC-MS可以从内源性代谢物的变化过程反应CRSsNP的发生情况;(2)“益气通窍饮”通过对差异性代谢物调控,对肺脾气虚型CRSsNP有一定的治疗作用。Objective To explore the differences in endogenous metabolites in the nasal mucosa of patients with chronic rhinosinusitis of Lung and Spleen-Qi deficiency syndrome using UPLC-MS(Ultra-high Liquid Chromatograph-Mass Spectrometer)metabolomics techniques compared to control group,and the changes of specific endogenous metabolites after drinking the decoction of“Yiqitongqiao Yin”thus to reveal the material basis for the identification and therapeutic target of chronic rhinosinusitis of Lung and Spleen-Qi deficiency type.Method From April 2019 to October 2021,32 patients with Lung and Spleen-Qi deficiency CRSsNP(treatment group)and 20 patients with control group were selected.UPLC-MS was used to detect endogenous metabolites in nasal mucosa of the two groups,and potential markers were screened and metabolic pathways were analyzed.Results There were 20significantly different metabolites and 5 metabolic pathways in the nasal mucosa of volunteers in the Lung and Spleen-Qi deficiency syndrome group compared to the control group.After eight weeks of oral decoction“Yiqitongqiao Yin”,the differential metabolites showed callback.Conclusion(1)The metabolomics method based on UPLC-MS can reflect the occurrence of CRSsNP from the changes of endogenous metabolites well;(2)“Yiqitongqiao Yin”has a certain therapeutic effect on CRSsNP of Lung and Spleen-Qi deficiency type by regulating the differential metabolites.
