耳内镜下双氧水冲洗联合曲安奈德益康唑治疗霉菌性外耳道炎的疗效及复发情况分析

摘　　要：目的探讨耳内镜下双氧水冲洗联合曲安奈德益康唑治疗霉菌性外耳道炎的疗效及复发情况。方法选取2019年9月~2020年9月在本院确诊的霉菌性外耳道炎患者120例,采用简单随机分组将患者分为观察组(n=60)和对照组(n=60)。对照组采用采用耳内镜下双氧水冲洗联合达克宁治疗,观察组采用耳内镜下双氧水冲洗联合曲安奈德益康唑治疗,均持续治疗3周。治疗后,评估两组患者临床疗效。检测两组血清C-反应蛋白(CRP)、肿瘤坏死因子(TNF-α)及白介素-6(IL-6)水平;记录两组治疗期不良反应发生情况;统计两组随访1个月、3个月、6个月的复发率。结果观察组霉菌性外耳道炎患者治疗有效率为91.67%,高于对照组的78.33%(P<0.05);观察组霉菌性外耳道炎患者血清CRP、TNF-α及IL-6水平均显著低于对照组,差异显著(P<0.05);两组总不良反应发生率分别为11.67%、20.00%,无显著差异(P>0.05);随访1、3、6个月观察组总复发率为5.00%,低于对照组的26.67%(P<0.05)。结论耳内镜下双氧水冲洗联合曲安奈德益康唑治疗霉菌性外耳道炎疗效确切,安全性高,复发率较低。Objective To explore the curative effect of otoscopic hydrogen peroxide flushing combined with Triamcinolone Acetonide and Econazole Nitrate Cream on fungal otitis externa and its recurrence.Methods A total of 120 patients confirmed with fungal otitis externa in the hospital were enrolled between September 2019 and September 2020.According to simple random grouping method,they were divided into observation group(n=60)and control group(n=60).The control group was treated with otoscopic hydrogen peroxide flushing and Miconazole Nitrate Cream,while observation group was treated with otoscopic hydrogen peroxide flushing and Triamcinolone Acetonide and Econazole Nitrate Cream for 3 weeks.After treatment,clinical curative effect in both groups was evaluated.The levels of serum C-reactive protein(CRP),tumor necrosis factor(TNF-α)and interleukin-6(IL-6)in both groups were detected.The occurrence of adverse reactions during treatment in both groups was recorded.The recurrence rate after 1 month,3months and 6 months of follow-up in both groups was statistically analyzed.Results The response rate of treatment in observation group was higher than that in control group(91.67%vs 78.33%)(P<0.05).The levels of serum CRP,TNF-αand IL-6 in observation group were significantly lower than those in control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(11.67%vs 20.00%)(P>0.05).The total recurrence rate after 1 month,3 months and 6 months of follow-up in observation group was lower than that in control group(5.00%vs 26.67%)(P<0.05).Conclusion The curative effect of otoscopic hydrogen peroxide flushing combined with Triamcinolone Acetonide and Econazole Nitrate Cream is significant on fungal otitis externa,with high safety and low recurrence rate.
