清窦汤用于老年慢性鼻-鼻窦炎伴鼻息肉患者鼻内镜术后的价值研究

摘　　要：目的研究清窦汤用于老年慢性鼻-鼻窦炎伴鼻息肉(CRSwNP)患者鼻内镜术(ESS)后的价值。方法选取我院104例行ESS手术的老年CRSwNP患者随机分为观察组与对照组,各52例。对照组行Messerklinger式ESS手术治疗,术后采用常规恢复治疗,观察组在对照组基础上加服清窦汤,两组均持续治疗4周。观察两组临床疗效,观察两组术前、术后4周鼻窦炎症状视觉模拟量表(VAS)症状评分,统计两组术后术腔清洁时间及术腔上皮化时间,评估两组术前、术后4周鼻黏膜功能(鼻黏液纤毛清除率、鼻黏液纤毛清除速度),测定两组术前、术后4周血清炎症因子水平[白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)]。结果观察组总有效率为90.38%,显著高于对照组的73.08%(P<0.05)。术后4周观察组鼻塞、鼻漏脓涕、头面部疼痛、嗅觉障碍评分显著低于本组术前及对照组术后4周(P<0.05)。观察组术腔清洁时间、术腔上皮化时间均显著低于对照组(P<0.05)。术后4周观察组鼻粘液纤毛清除率、鼻粘液纤毛清除速度显著高于本组术前及对照组术后4周(P<0.05)。术后4周观察组IL-6、IL-8、TNF-α水平显著低于本组术前及对照组术后4周(P<0.05)。结论清窦汤用于老年CRSwNP患者ESS术后具有较好疗效,可显著减少患者术后术腔清洁时间及术腔上皮化时间,改善鼻黏膜功能,降低机体炎症反应。Objective To study the value of qingdou decoction after endoscopic sinus surgery(ESS)in elderly patients with chronic rhinosinusitis and nasal polyps(crswnp).Methods 104 elderly patients with crswnp who underwent ess in our hospital were randomly divided into observation group and control group,52 cases in each group.The control group was treated with messerklinger ess,and the patients in the control group were treated with routine recovery therapy,The observation group was given qingdou Decoction on the basis of the control group,and both groups were treated for 4 weeks.The clinical effects of the two groups were observed,The symptom scores of visual analogue scale(VAS)were observed before and 4 weeks after operation,The cleaning time and epithelization time of the two groups were counted,The nasal mucosa function(nasal mucociliary clearance rate,nasal mucociliary clearance rate)was evaluated before and 4 weeks after operation,The levels of serum inflammatory factors(IL-6,IL-8,TNF-α)were measured before and 4 weeks after operation-α(TNF-α).Results The total effective rate of the observation group was 90.38%,which was significantly higher than 73.08%of the control group(P<0.05).At 4 weeks after operation,the scores of nasal obstruction,rhinorrhea,purulent discharge,head and face pain and olfactory dysfunction in the observation group were significantly lower than those before operation and 4 weeks after operation in the control group(P<0.05).The cleaning time and epithelization time of the observation group were significantly lower than those of the control group(P<0.05).The nasal mucociliary clearance rate and nasal mucociliary clearance speed of the observation group were significantly higher than those of the control group and the observation group at 4 weeks after operation(P<0.05).The levels of IL-6,IL-8 and TNF in the observation group were higher than those in the control group 4 weeks after operation-αThe level was significantly lower than that before operation and 4 weeks after operation in the contr
