茯苓温肾胶囊对异丙肾上腺素诱导心力衰竭模型大鼠心肌细胞及心室重构的影响

摘  要:目的:探讨茯苓温肾胶囊对异丙肾上腺素诱导心力衰竭模型大鼠心肌细胞及心室重构的影响,并分析其作用机制。方法:31只大鼠随机分为空白组(7只)、模型组(8只)、中药组(8只)、西药组(8只),除空白组外,其余大鼠均采用过量异丙肾上腺素建立心衰模型。造模成功后,空白组、模型组生理盐水灌胃,西药组予以依那普利片处理,中药组予以茯苓温肾胶囊处理。通过ELISA血清学检测及病理切片分析,观察各组细胞因子血清表达水平及心肌形态、结构变化。结果:与空白组相比,模型组大鼠血清N末端脑钠肽(NT-proBNP)、基质裂解素2(ST2)、白介素6(IL-6)、肿瘤坏死因子(TNF-α)、基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)水平均明显升高(P<0.01),心肌细胞排列紊乱,纤维组织大量增生,大量炎性细胞浸润。与模型组相比,中药组和西药组均可不同程度地降低血清NTproBNP、ST2、IL-6、TNF-α、MMP-2、MMP-9水平(P<0.01),心肌细胞排列相对整齐、纤维组织增生减轻,少量炎性细胞浸润。结论:茯苓温肾胶囊可调控心肌细胞代谢,改善心肌细胞肥大程度,抑制心室重构,达到治疗心衰的目的,其机制与阻断β-肾上腺素能受体和炎症反应之间的串扰、调节心肌基质金属蛋白酶的活化有关。Objective:To explore the effect of Fuling Wenshen Capsule on myocardial cell and ventricular remodeling in model rats with heart failure induced by isoproterenol and analyze the mechanism.Methods:31 rats were randomly divided into blank group(7 rats),model group(8 rats),Chinese medicine group(8 rats)and western medicine group(8 rats).All rats except the rats in the blank group were treated with excessive isoproterenol to establish heart failure models.After the modeling is successful,the blank group and model group were given normal saline by gavage,the western medicine group was given enalapril tablet,and the Chinese medicine group was given Fuling Wenshen Capsule.Serum expression levels of cytokines and changes in myocardial morphology and structure were observed by ELISA and pathological section analysis.Results:Compared with the blank group,rats serum NT-proBNP,ST2,IL-6,TNF-α,MMP-2,and MMP-9 levels were significantly increased in the model group(P <0.01).The cardiomyocyte arrangement was disturbed,with massive proliferation of fibrous tissue and massive inflammatory cell infiltration.Compared with the model group,the levels of serum NT-proBNP,ST2,IL-6,TNF-α,MMP-2 and MMP-9 in the Chinese medicine group and the western medicine group were decreased to different degrees(P<0.01).The myocardial cells were arranged in a relatively neat manner,the fibrous tissue hyperplasia was reduced,and a small amount of inflammatory cells infiltrated.Conclusion:Fuling Wenshen Capsule can regulate cardiomyocyte metabolism,improve the degree of cardiomyocyte hypertrophy,inhibit ventricular remodeling,and achieve the purpose of treating heart failure.The mechanism is related to blocking the interference between β-adrenergic receptors and inflammatory response and regulating the activation of myocardial matrix metalloproteinases.
