摘 要:目的:观察四子温肺汤对慢性阻塞性肺疾病(COPD)痰湿蕴肺型模型大鼠气道炎症与黏液高分泌的影响及其作用机制。方法:40只SPF级雄性SD大鼠随机分成4组,分别为正常组、模型组、氨溴索组及四子温肺汤组,每组10只。通过脂多糖(LPS)气道内滴注法+烟熏+番泻叶浸液灌胃方法制备COPD痰湿蕴肺型大鼠模型。氨溴索组以0.93 g/(mL·100 g)氨溴索混悬液灌胃,四子温肺汤组以0.87 mg/(mL·100 g)四子温肺汤灌胃,空白组、模型组均给予等体积的生理盐水每日灌胃一次。分类计数大鼠肺泡灌洗液(BALF)中炎症细胞;ELISA法检测各组大鼠BALF中白介素-13(IL-13)、白介素-8(IL-8)、细胞间黏附分子-1(ICAM-1)、黏蛋白5AC(MUC5AC)和黏蛋白5B(MUC5B)的含量。结果:模型组与空白组比较,大鼠BALF中炎症细胞数量及IL-13、IL-8、ICAM-1、MUC5AC、MUC5B的含量均明显升高(P<0.01);氨溴索组、四子温肺汤组与模型组比较,大鼠BALF中炎症细胞数量及IL-13、IL-8、ICAM-1、MUC5AC、MUC5B的含量均明显下降(P<0.01);四子温肺汤组与氨溴索组比较,大鼠BALF中炎症细胞数量及IL-13、IL-8、ICAM-1、MUC5AC、MUC5B的含量均减少(P<0.05)。结论:四子温肺汤对COPD痰湿蕴肺型大鼠有较佳的治疗效果,能减轻气道炎性反应,还具有抑制气道黏液高分泌的作用,且其机制可能与通过抑制IL-13表达,进而抑制巨噬细胞及杯状细胞活性有关。Objective:To observe the effects and mechanism of Sizi Wenfei Decoction on airway inflammation and mucus hypersecretion in model rats with chronic obstructive pulmonary disease(COPD)due to phlegm-dampness amassing in lung.Methods:40 SPF-grade male SD rats were randomly divided into four groups,namely,normal group,model group,ambroxol group and Sizi Wenfei Decoction group,with 10 rats in each group.The COPD rats model with phlegm-dampness amassing in lung was prepared by lipopolysaccharide(LPS)intra-airway drip,fumigation and senna leaf infusion gavage.The ambroxol group was gavaged with 0.93 mg/(mL·100 g) ambroxol suspension,and the Sizi Wenfei Decoction group was gavaged with 0.87 g/(mL·100 g) of Sizi Wenfei Decoction.The normal group and model group were gavaged with equal volume of saline once a day.The inflammatory cells in bronchoalveolar lavage fluid(BALF)of rats were sorted and counted;the contents of interleukin-13(IL-13),interleukin-8(IL-8),intercellular adhesion molecule-1(ICAM-1),mucin5AC(MUC5AC)and mucin5B(MUC5B)in BALF of rats in each group were detected by ELISA.Results:Compared to the normal group,the number of inflammatory cells and the contents of IL-13,IL-8,ICAM-1,MUC5AC and MUC5B in BALF of rats were significantly higher in the model group(P<0.01);compared to the model group,the number of inflammatory cells and the contents of IL-13,IL-8,ICAM-1,MUC5AC and MUC5B in BALF of rats were significantly lower in the ambroxol and Sizi Wenfei Decoction groups(P<0.01);compared to the ambroxol group,the number of inflammatory cells and the contents of IL-13,IL-8,ICAM-1,MUC5AC and MUC5B in BALF of rats were significantly decreased in the group of Sizi Wenfei Decoction compared with the group of ambroxol(P<0.05).Conclusion:Sizi Wenfei Decoction has a better therapeutic effect on COPD rats due to phlegm-dampness amassing in lung.It can reduce the airway inflammatory response,and also has the effect of inhibiting airway mucus hypersecretion,which may be related to the inhibition of IL-13 expression and then
