摘  要:目的:研究股骨粗隆间骨折外固定架近端不同固定方式固定的生物力学性能。方法:对志愿者骨盆及股骨全长行CT扫描,将CT数据文件导入Mimics10.0软件进行三维重建处理,并以IGES文件形式保存,导入逆向工程软件(Geomagic Studio10.0软件),构建完整的股骨及外固定架的三维几何模型,利用ANSYS软件建立EvansⅠ型股骨粗隆间骨折模型,在ANSYS软件中赋予不同的弹性模量及泊松比,建立近端交叉、内聚、平行三种固定方式,并在模型中进行组装,予股骨头顶端施加载荷,分析其固定后的应力分布情况。结果:近端3种不同固定方式其最大应力均出现在股骨近端内侧2号螺钉与骨折交界处,其最大值出现在平行固定方式中,为119.33 MPa,此载荷与螺钉材料属性所能承受的最大载荷相比较小;内聚方式整体应力远小于平行与交叉固定方式。结论:外固定架近端3种固定方式均可有效治疗稳定性股骨粗隆间骨折,但内聚型更符合人体的生物力学要求。Objective:To study the biomechanical properties of different fixation methods of proximal external fixator for intertrochanteric fracture.Methods:CT scan was performed on the pelvis and femur of the volunteers.Then the CT data files were imported into Mimics10.0 software for 3D reconstruction and saved in the form of IGES files.Then they were imported into GeoMagic Studio10.0 software to construct 3D geometric models of the complete femur and external fixator.The Evans I intertrochanteric fracture model of femur was established by using ANSYS software.Different elastic modulus and Poisson′ s ratio were given in ANSYS software,and three fixation methods of proximal crossover,cohesion and parallel were established,and then assembled in the model.A load was applied to the tip of the femoral head and the stress distribution after fixation was analyzed.Results:The maximum stress of the three different fixation methods appeared at the junction between the No.2 screw and the fracture of the proximal femur,and the maximum stress appeared in the parallel fixation method,which was 119.33 Mpa.It was smaller than the maximum load that the screw material property can bear.The overall stress of cohesive fixation is much less than that of parallel and cross fixation.Conclusion:The three fixation methods of proximal external fixator can effectively treat stable intertrochanteric fractures,while the cohesive type is more in line with the biomechanical requirements of human body.
