健脾化瘀法治疗胰岛素抵抗型多囊卵巢综合征临床观察

摘　　要：目的:观察健脾化瘀法治疗脾虚痰瘀型胰岛素抵抗型多囊卵巢综合征(PCOS)的临床疗效。方法:100例患者随机分为治疗组和对照组,每组50例,对照组服用达英-35、二甲双胍,治疗组在西药基础上加服健脾化瘀中药;治疗3个月经周期,停药后1个月观察临床疗效。通过组间及组内治疗前后中医临床症状积分、卵巢体积变化及黄体生成素(luteinizing hormone,LH)、睾酮(testosterone,T)、空腹胰岛素(fasting insulin fins,FINS)的变化来评价治疗效果。结果:治疗组临床有效率明显高于对照组(P<0.05);两组患者治疗后症状积分均明显下降,卵巢体积均有不同程度缩小,组间比较差异具有统计学意义(P<0.05);经治疗后两组患者LH、T、FINS值均较本组治疗前明显改善,治疗后LH组间比较差异具有统计学意义(P<0.05)。结论:健脾化瘀法治疗胰岛素抵抗型PCOS患者可纠正胰岛素抵抗状态,改善临床症状。Objective:To observe the clinical efficacy of the method of invigorating spleen and removing blood stasis in the treatment of insulin-resistant polycystic ovarian syndrome due to spleen deficiency and phlegm stasis.Methods:The patients were randomly divided into treatment group and control group,with 50 cases in each group.Patients in the control group took Daying-35 and metformin,while patients in the treatment group took Chinese herbs of strengthening spleen and removing blood stasis in addition to Western medicines.The clinical efficacy was observed for 3 menstrual cycles and 1 month after discontinuation of the drug.The treatment effects were evaluated by the pre-and post-treatment TCM clinical symptom scores,ovarian volume changes and changes in luteinizing hormone(LH),testosterone(T)and fasting insulin fins(FINS)between groups and within groups.Results:The clinical efficiency of the treatment group was significantly higher than that of the control group(P<0.05);The treated patients all had a significant decrease in symptom scores and different degrees of ovarian volume reduction,with statistically significant differences between groups(P <0.05);After treatment,LH,T and FINS values improved significantly compared with those before treatment,and the difference between groups after LH treatment was statistically significant(P<0.05).Conclusion:The treatment of insulinresistant PCOS patients with the method of strengthening the spleen and removing blood stasis can correct the insulin resistance status and improve clinical symptoms.
