彭斌内外合治肺肾虚寒型变态反应性鼻-鼻窦炎经验

摘　　要：彭斌教授认为,变态反应性鼻-鼻窦炎病机为肺、脾、肾三脏虚损,正气不足,腠理疏松,卫表不固,遇寒而发病,宜标本兼治,补益肺肾,温阳散寒,常用温阳祛风汤加减联合穴位贴敷治疗。温阳祛风汤温补脾肾,祛风散寒;穴位贴敷补益肺肾,散寒通窍。若伴咳嗽,可加桔梗、紫菀止咳化痰,降逆肺气;若睡眠不佳,可加酸枣仁、茯神安神助眠;若头痛头晕,可加磁石、龙骨、天麻平肝抑阳;若自汗出,加黄芪、白术补气固表止汗。另外,应尽量避免接触过敏介质,注意保暖,坚持长期服药。Professor Peng Bin believes that the pathogenesis of allergic rhinosinusitis is the deficiency of the lung, spleen and kidney, the lack of healthy qi,the loose interstitial striae, the unstable defense exterior, and the onset of cold.It is suitable to treat manifestation and root cause concurrently, tonify the lung and kidney, warm yang and dissipate cold.So Modified Decoction for Warming Yang and Eliminating Wind combined with acupoint application are commonly used.Decoction for Warming Yang and Eliminating Wind can warm the spleen and kidney, expel wind and disperse cold.Acupoint application is beneficial to the lung and kidney, dispersing cold and dredging the orifices.If accompanied by cough, Jiegeng(Radix Platycodonis) and Ziwan(Radix et Rhizoma Asteris) can be added to relieve cough and resolve phlegm, and descend upward perversion of lung qi.For poor sleep, Suanzaoren(Semen Ziziphi Spinosae) and Fushen(Sclerotium Poriae Pararadicis) can be added to calm the spirit and improve sleep.For headache and dizziness, Cishi(Magnetitum),Longgu(Os Draconis) and Tianma(Rhizoma Gastrodiae) can be added to calm the liver and suppress yang.For spontaneous sweating, Huangqi(Radix Scutellariae) and Baizhu(Rhizoma Atractylodis Macrocephalae) can be added to replenish qi,consolidate the surface and stop sweating.In addition, it is necessary to try to avoid contact with allergic media, pay attention to keeping warm, and adhere to long-term medication, etc.
