摘 要:目的:观察按压心俞穴对心肌缺血模型大鼠细胞凋亡相关蛋白的影响,并初步探讨其作用机制。方法:将60只SPF级健康雄性SD大鼠随机分为空白组、模型组、按法组和盐酸地尔硫卓组,每组15只。除空白组外,其余组大鼠建立心肌缺血模型。造模完成后,按法组大鼠于双侧心俞穴上进行按压刺激,盐酸地尔硫卓组大鼠腹腔注射盐酸地尔硫卓(1.5 mg·kg-1),每天1 d,连续干预7 d,空白组和模型组不予处理。心电图监测J点位移变化值;HE染色检测心肌组织病理形态学改变;TUNEL染色检测心肌细胞凋亡率;ELISA检测大鼠血清中肌酸激酶同工酶MB(creatine kinase, MB form, CK-MB)、心肌肌钙蛋白-Ⅰ(cardiac troponin-Ⅰ,cTn-Ⅰ)的含量;Western Blot检测大鼠心肌组织活化的半胱氨酰天冬氨酸特异性蛋白酶3(cleaved cysteinyl aspartate-specific protease 3,C-Caspase 3)、B淋巴细胞瘤-2基因(B-cell lymphoma-2,Bcl-2)、Bcl-2相关X蛋白(Bcl-2-associated X,Bax)的蛋白表达水平。结果:与空白组比较,模型组J点位移变化值、心肌细胞凋亡率及CK-MB、cTn-Ⅰ含量升高,Bax、C-Caspase 3蛋白表达水平上调,Bcl-2蛋白表达水平下调;与模型组比较,按法组J点位移变化值、心肌细胞凋亡率及CK-MB、cTn-Ⅰ含量下降,Bax、C-Caspase 3蛋白表达水平下调,Bcl-2蛋白表达水平上调,差异均有统计学意义(P<0.05)。HE染色显示:按法能改善心肌纤维的纤维化状态,减少心肌细胞坏死程度。结论:按压心俞穴可改善心肌缺血损伤,其作用机制与抑制心肌细胞凋亡有关。Objective: To observe the effect of pressing Xinshu(BL 15) on apoptosis-related protein in rats with myocardial ischemia and explore its mechanism.Methods: A total of 60 SPF grade healthy male SD rats were randomly divided into the blank group, the model group, the pressing manipulation group and the diltiazem hydrochloride group, with 15 rats in each group.Except the blank group, myocardial ischemia models were established in the other groups.After the completion of modeling, the rats in the method group were pressed and stimulated on the bilateral Xinshu points.The rats in the diltiazem hydrochloride group were injected intraperitoneally with diltiazem hydrochloride(1.5 mg·kg-1),once a day, seven times in total.The blank group and the model group were not be processed.ECG was used to monitor the change valueof J displacement.HE staining was used to detect the pathological changes of myocardial tissue.TUNEL staining was used to detect the apoptosis rate of myocardial cells.The contents of creatine kinase isoenzyme MB form(CK-MB) and cardiac troponin Ⅰ(cTn-Ⅰ) in serum of rats were detected by ELISA.Western Blot was used to detect the levels of activated cysteinyl-aspartate-specific protease 3(C-Caspase 3),B-cell lysoma-2(Bcl-2),and Bcl-2 associated X protein expression(Bax) in rat myocardium.Results: Compared with the blank group, the change value of J point, the rate of cardiomyocyte apoptosis and the contents of CK-MB and cTn-Ⅰ in the model group increased, the expression levels of Bax and C-Caspase 3 protein up-regulated, and the expression level of Bcl-2 protein decrea down-rugulated.Compared with the model group, the change value of J point, the rate of cardiomyocyte apoptosis and the contents of CK-MB and cTn-Ⅰ in the method group decreased, the expression levels of Bax and C-Caspase 3 protein down-regulated, and the expression level of Bcl-2 protein up-regulated, and all the differences were statistically significant(P<0.05).HE staining showed that pressing manipulation could improve the fibrosis
