穴位贴敷配合中药汤剂治疗慢性再生障碍性贫血临床研究

摘　　要：目的:观察穴位贴敷配合中药汤剂治疗慢性再生障碍性贫血的临床疗效。方法:将60例慢性再生障碍性贫血患者按照随机数字表法分为常规组和试验组,每组各30例。常规组给予穴位贴敷疗法,试验组在常规治疗的基础上联合中药汤剂治疗。比较两组患者的临床疗效、不良反应发生率及治疗前后白细胞、血红蛋白、血小板、血清炎性因子水平[白细胞介素-8(interleukin-8,IL-8)、干扰素-γ(interferon-γ,IFN-γ)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)]、中医证候积分、端粒长度及Dyskerin mRNA表达水平变化情况。结果:常规组有效率为73.33%,试验组有效率为93.33%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后白细胞、血红蛋白、血小板高于本组治疗前,IL-8、IFN-γ、TNF-α水平低于本组治疗前,且治疗后组间比较,差异具有统计学意义(P<0.05)。两组患者治疗后中医证候积分低于本组治疗前,且治疗后试验组低于常规组,差异具有统计学意义(P<0.05)。两组患者治疗后端粒长度,Dyskerin mRNA表达水平低于本组治疗前,且治疗后试验组低于常规组,差异具有统计学意义(P<0.05)。试验组不良反应发生率低于常规组,差异具有统计学意义(P<0.05)。结论:穴位贴敷配合中药汤剂治疗慢性再生障碍性贫血,可改善患者白细胞、血红蛋白、血小板及血清炎性因子水平,促进骨髓造血功能的恢复。Objective: To observe the clinical efficacy of acupoint application combined with Chinese medicinal decoction on chronic aplastic anemia.Methods: A total of 60 patients with chronic aplastic anemia were divided into the conventional group and the experimental group according to random number table method, with 30 cases in each group.The conventional group was treated with acupoint application therapy, while the experimental group was treated with Chinese medicinal decoction on the basis of the routine treatment.The clinical efficacy, the incidence of adverse reactions, and the changes of the levels of white blood cells, hemoglobin, platelets, and serum inflammatory factors [including interleukin-8(IL-8),interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α)],TCM syndrome score, telomere length and Dyskerin mRNA expression of the two groups before and after treatment were compared.Results: The effective rate was 73.33% in the conventional group and 93.33% in the experimental group.The difference between the two groups was statistically significant(P<0.05).The white blood cells, hemoglobin and platelets of patients in both groups after treatment were higher than those of the same group before treatment, while IL-8,IFN-γ and TNF-α were lower than those of the same group before treatment, and all the differences between the groups were statistically significant(P<0.05).After treatment, the TCM syndrome score of the two groups was lower than that of the same group before treatment, and the experimental group′s score was lower than that of the conventional group, and the difference was statistically significant(P<0.05);The telomere length and Dyskerin mRNA expression level in the two groups were lower than those before treatment, and the experimental group was lower than that of the conventional group after treatment, and all the differences were statistically significant(P<0.05).The incidence of adverse reactions in the experimental group was lower than that in the conventional group, and the difference was statis
