髓内钉系统与空心螺钉治疗骨盆前环骨折的疗效

摘　　要：目的对比耻骨上支髓内钉系统与耻骨上支空心螺钉治疗骨盆前环骨折的临床疗效。方法回顾性分析2021年1—6月新疆维吾尔自治区人民医院创伤骨科收治的64例耻骨上支骨折的患者资料,根据内固定方式不同分组:观察组(髓内钉系统)32例,其中男18例,女14例,平均年龄(49.52±5.48)岁;对照组(空心螺钉)32例,男20例,女12例,年龄(52.43±6.37)岁。比较两组患者的手术时间、术中出血量、术中X线透视次数、骨折愈合时间,术后骨折复位情况,应用Matta放射学标准评价复位质量,采用Majeed评分评价骨盆功能恢复程度。结果观察组患者术中透视次数显著小于对照组,差异有统计学意义(P<0.05);观察组术后Matta放射学标准评定优良率显著高于对照组,差异有统计学意义(P<0.05)。观察组患者手术时间、手术出血量、骨折愈合时间、末次随访Majeed评分与对照组相比,差异无统计学意义(P>0.05)。结论应用骨盆前柱髓内钉系统治疗耻骨上支骨折的临床疗效肯定,操作微创,并发症发生率低,可作为耻骨上支骨折的优选方案。Objective In the treatment of pelvic anterior ring fractures,the clinical efficacy of superior pubic branch intramedullary nail system and superior pubic branch hollow screw was compared.Methods The data of 64 patients with superior pubic branch fracture treated in the Department of Orthopaedics,people′s Hospital of Xinjiang Uygur Autonomous region from January 2021 to June 2021 were analyzed retrospectively.here were18 males and 14 females in the study,with an average age of(49.52±5.48)years,and 32 patients in the control group(hollow screws),with an average age of(52.43±6.37)years.The two groups were compared in terms of operation time,intraoperative blood loss,intraoperative X-ray frequency,fracture healing time,and postoperative fracture reduction.Matta radiology standard was used to evaluate the reduction quality,and Majeed score was used to evaluate the recovery of pelvic function.Results The observation group had significantly fewer intraoperative fluoroscopies than the control group,and the observation group had a significantly higher rate of excellent and good postoperative Matta radiology standards than the control group.There was no significant difference in the average operation time,operative blood loss,fracture healing time and Majeed score of the last follow-up between the observation group and the control group.Conclusion The application of pelvic anterior column intramedullary nail system in the treatment of superior pubic branch fracture has a clear clinical effect,is a minimally invasive procedure,and has a low risk of complications.It is the best procedure for superior pubic branch fracture.
