三种不同切口内固定治疗跟骨骨折的临床研究

摘　　要：目的探讨跗骨窦切口、外侧斜形切口与外侧L形切口内固定治疗跟骨骨折的临床效果。方法按随机数字表法将2018年6月-2021年8月本院收治的跟骨骨折患者(n=96)分为三个组别,即跗骨窦切口组、外侧斜形切口组及外侧L形切口组各32例,对比各组术中及术后恢复情况,统计各组术后并发症发生率进行对比。结果跗骨窦切口组、外侧斜形切口组在术中出血量、手术时间、切口愈合时间以及住院时间等方面均短于外侧L形切口组,差异有统计学意义(P<0.05);跗骨窦切口组、外侧斜形切口组在术后并发症发生率均低于外侧L形切口组,差异有统计学意义(P<0.05)。结论经跗骨实和外侧斜形微创切口手术入路较常用外侧扩大“L”切口具有疗效更优,创伤小,手术及住院时间、切口愈合时间更短及切口并发症发生率更小的优势,值得临床推广应用。Objective To discuss the clinical effect of sinuses tarsi incision,lateral oblique incision and lateral L-shaped incision with internal fixation in calcaneal fracture.Methods 96 calcaneal fracture patients treated from June 2018 to August 2021 in our department were selected and randomly assigned to sinuses tarsi incision group,lateral oblique incision group and lateral L-shaped incision group,32 cases in each group.The intraoperative and postoperative rehabilitation conditions and complication rate were compared.Results Compared with lateral L-shaped incision group,the intraoperative blood loss,operation time,wound healing time and hospitalization time in sinuses tarsi incision group and lateral oblique incision group were dramatically reduced(P<0.05);the complication rate in sinuses tarsi incision group and lateral oblique incision group was significantly lower than that of lateral L-shaped incision group(P<0.05).Conclusion Compared with lateral L-shaped incision,the minimally invasive sinuses tarsi incision and lateral oblique incision has better efficacy,less trauma,shorter operation time,hospitalization time and would healing time,and lower complication rate.It is worthy of clinical promotion and application.
