摘 要:目的考察计划—执行—检查—行动(PDCA)方法构建基于哈贝马斯交往行动理论的新型医患沟通模式对医患关系的影响。方法以常州市第一人民医院儿科医护人员及患者为研究对象,于2020年1~12月通过实施PDCA方法构建基于哈贝马斯交往行动理论的新型医患沟通模式。纳入儿科医生8名,护士18名。2019年1~12月实施新型医患沟通模式前于儿科就诊的患儿家属60例。2020年1~12月实施新型医患沟通模式后于儿科就诊的患儿家属60名。比较新型医患沟通模式实施前后患儿家属对医护人员满意程度、患儿家属照顾能力并统计2019年和2020年医院医患纠纷数目。结果与实施前相比,新型医患沟通模式实施后初入院时接诊护士、住院期间护士健康教育和康复指导、初入院时接诊医生、住院期间医生沟通解释病情和住院期间医生诊疗措施和治疗效果的满意程度均显著提高(P<0.05)。相比于实施前,新型医患沟通模式实施后患儿家属照顾知识、能力和技术均显著提高(P<0.05)。相比于2019年(实施前),2020年(实施后)医患纠纷数明显降低,从325例下降至268例,降幅为17.54%,医患沟通项显著降低,差异具有统计学意义(P<0.05)。结论PDCA方法构建基于哈贝马斯交往行动理论的新型医患沟通模式有助于降低医疗纠纷发生率,提高患者满意度,可在临床上推广应用。Objective To investigate the influence of PDCA method to construct a new doctorpatient communication model based on Habermas communicative action theory on doctor-patient relationship.Methods Taking the pediatric medical staff and patients in Changzhou First People's Hospital as the research object,in January 2020 to December 2020 to implement PDCA method to construct new doctor-patient communication model based on Habermas'theory of communicative action.8 pediatricians and 18 nurses were included.From January to December 2019,60 children's family members visited the pediatric department before the implementation of the new doctor-patient communication mode.From January to December 2020,60 children’s family members visited the pediatric department after the implementation of the new doctor-patient communication mode.The compared before and after the implementation of the new doctor-patient communication mode,and the number of doctor-patient disputes in hospitals in 2019 and 2020 were analyzed.Results Compared with before carrying out,after the introduction of a new mode of doctor-patient communication early on admission accepts nurses,hospital nurse health education and rehabilitation guidance,first hospital doctors,hospital doctor communication to explain the disease and hospital a doctor diagnosis and treatment measures and treatment effect of satisfaction were significantly improved(P<0.05).Compared with before the implementation of the new doctor-patient communication mode,the care knowledge,ability and technology of children's families were significantly improved after the implementation of the new doctor-patient communication mode(P<0.05).Compared with 2019(before implementation),the number of doctor-patient disputes decreased significantly in 2020(after implementation),from 325 cases to 268 cases,with a decrease of 17.54%.Doctor-patient communication items were significantly reduced,and the difference was statistically significant(P<0.05).Conclusion PDCA method to construct a new doctor-patient communica
