基于“君火不煦脾土”理论探究心血管疾病与非酒精性脂肪肝共病的发病机制

摘　　要：随着人民生活水平的提升与生活节奏的加快,心血管代谢性疾病的发病率逐年升高。其中,心血管疾病(cardiovascular disease,CVD)与非酒精性脂肪肝(non-alcoholic fatty liver disease,NAFLD)共病的占比居高不下,严重威胁人类健康。中医药整体观和辨证论治的特点,使其在探究共病共性方面展现出了独特优势。本文以“君火不煦脾土”立论,探究CVD与NAFLD共病的形成机制,认为心火温煦之力不足,脾土失温,难化水饮,痰瘀中困是其主要病机。通过整合“君火不煦”“脾土失温”的现代科学研究进展,挖掘“君火不煦脾土”理论指导下CVD与NAFLD的共病发病机制,并总结了补火助阳法治疗NAFLD的相关报道。从“君火不煦脾土”病机以及温通心阳治法的角度,为CVD与NAFLD共病的防治提供依据。With the improving living standards and the accelerating pace of life,the incidence of cardiovascular metabolic diseases has increased year by year.The proportion of comorbidity of cardiovascular disease(CVD) and nonalcoholic fatty liver disease(NAFLD) remains high,which are serious hazards to human health.The holism view and the characteristics of syndrome differentiation of traditional Chinese medicine make it show its unique advantages in exploring the common features of multimorbidity.To explore the pathogenesis of the CVD and NAFLD comorbidities,we put forward the theory of " monarch fire does not warm spleen soil".Because of the failure of the monarch fire warmth to the spleen earth,phlegm and blood stasis were generated.The article introduces the modern scientific research of " monarch fire fails to warm" and " spleen earth keep damp" and its latest developments in a tidy mind and excavates the pathogenesis of the CVD and NAFLD comorbidities.And we also summarize reports of the method of tonifying fire and assisting Yang to cure NAFLD to provide the basis for the prevention and cure of the comorbidity of CVD and NAFLD.
