摘 要:目的 主要探讨胎儿无包块型脊柱裂的超声诊断。方法 回顾性分析行胎儿系统结构筛查时的超声资料,应用凸阵探头观察胎儿脊柱形态、结构及周边组织关系进行辨认、判断,定位脊髓圆锥末端位置,对未见明显包块可疑脊柱裂、圆锥末端低的胎儿联合应用线阵探头进一步扫查,并与引产或分娩后的结果作对比。结果 共发现95例脊柱裂胎儿,其中无包块型脊柱裂17例,凸阵探头能发现9例,凸阵探头联合线阵探头能发现16例。凸阵探头、凸阵探头联合线阵探头对无包块型脊柱裂诊断的灵敏度分别为52.94%、94.12%,特异度均为99.97%。结论 凸阵探头联合线阵探头扫查可以发现微小病变,特别是对于无包块型、脊髓圆锥末端位置低的脊柱裂。Objective To evaluate the ultrasound diagnosis of fetal spina bifida without mass.Methods The ultrasound data of fetus who underwent structure screening in the second trimester in our hospital were retrospectively analyzed.Convex array probe was used to observe the morphology and structure of the fetal spine,and to identify and judge the relationship between the surrounding tissues,and the conus medullaris termination could be located.The fetuses without obvious mass who suspected to spina bifida and with lower conus medullaris termination position were scanned further with linear array probe and compared with the results of induced labor or delivery.Results A total of 95 fetuses with spina bifida were diagnosed,and 17 cases of spina bifida without mass were found.9 cases could be detected by convex array probe,and 16 cases could be detected by convex array probe combined with linear array probe.The sensitivity of convex array probe,and convex array combined with linear array probe diagnosing spina bifida without mass were 52.94% and 94.12% respectively,and the specificity of both was 99.97%.Conclusions Convex array probe combined with linear array probe can be used to detect minimal lesions,especially for spina bifida without mass at lower conus medullaris termination position.
