经阴道超声检查在中晚期妊娠合并宫颈疾病所致出血的诊断价值

摘　　要：目的 探讨经阴道超声检查对中晚期妊娠合并宫颈疾病所致出血的诊断价值。方法 回顾性分析在妊娠中晚期由于宫颈疾病所致出血的病例49例,依据妇科检查及术后病理结果,分析并总结超声声像图表现。结果 妊娠中晚期合并宫颈疾病共49例,根据不同病理学类型分为妊娠合并宫颈息肉25例,妊娠合并宫颈蜕膜息肉样病变20例,妊娠合并宫颈静脉曲张3例,妊娠合并宫颈癌1例,符合率为81.6%(40/49);漏诊5例,漏诊率为10.2%(5/49);误诊4例,误诊率为8.16%(4/49)。结论 妊娠期孕妇出现阴道流血症状,需经阴道超声检测关注宫颈疾病,对妊娠期出现不明原因阴道出血现象具有较高的鉴别诊断意义。Objective To investigate the diagnostic value of transvaginal ultrasonography in vaginal bleeding caused by cervical diseases during the second and third trimester of pregnancy.Methods A retrospective analysis was performed in 49 cases with vaginal bleeding caused by cervical diseases in the second and third trimester of pregnancy diagnosed and treated in our hospital.Based on the gynecological examination and the postoperative pathological results,the characteristics of the ultrasonogram were analyzed and summarized.Results The 49 cases were classified into 25 cases of cervical polyps during pregnancy,20 cases of cervical decidua-like polypoid lesions during pregnancy based on different pathological types,3 cases of cervical varices during pregnancy,and 1 case of cervical cancer during pregnancy.The coincidence rate was 81.6%(40/49),the missed diagnosis rate was 10.2%(5/49),and the misdiagnosis rate was 8.16%(4/49).Conclusions It is necessary to pay attention to cervical diseases for pregnant women with vaginal bleeding.A vaginal ultrasound examination should be ordered,which offers high diagnostic value in unexplained vaginal bleeding during pregnancy.
