立体定向仪在高血压脑出血手术治疗中的应用

摘　　要：目的:探讨立体定向仪在高血压脑出血手术治疗中的应用效果。方法:选取2017年12月—2021年12月周口市中医院收治的160例高血压脑出血手术治疗患者作为研究对象,分为观察组和对照组,每组各80例,对照组患者使用传统简易定位颅内血肿清除术,观察组患者使用立体定向下颅内血肿清除术。研究对比患者引流管留管时间,术后并发症发生率,住院时间,1 d、3 d、5 d血肿残留量;采用Glasgow结局分级、Barthel量表、美国国立卫生研究院卒中量表(NIHSS)评估患者神经功能缺损程度。结果:观察组患者各临床指标、后血肿残留量、临床预后评分及总治疗有效率均优于对照组,差异有统计学意义(t=14.998、26.920、14.473;2.564、12.065、3.191;7.839、5.028、6.112;χ^(2)=5.342,P<0.05)。结论:利用立体定向仪治疗高血压脑出血具有良好的效果,更利于血肿引流,减轻对脑组织的压迫损伤,使脑组织及早恢复血流灌注,有利于损伤神经的恢复,避免了后期出现较为严重的并发症而影响预后。Objective:To explore the application effect of stereotactic in the surgical treatment of hypertension cerebral hemorrhage.Methods:160 patients admitted to the hospital for surgical treatment of hypertensive cerebral hemorrhage from December2017 to December 2021 were selected as study subjects and divided into observation and control groups, with 80 cases in each group. The control group used traditional simple positioning intracranial hematoma removal, and the observation group used stereotactic subcranial hematoma removal. The duration of drainage tube retention, postoperative complication rate, length of stay, 1 d,3 d, and 5 d hematoma residuals were compared, and the degree of neurological deficit was assessed using the Glasgow Outcome Rating, the Barthel Scale, and the National Institutes of Health Stroke Scale(NIHSS).Results:All clinical indicators, posterior hematoma residual volume, clinical prognosis score and overall treatment efficiency were better in the observation group than in the control group, with statistically significant differences(t=14.998, 26.920, 14.473;2.564, 12.065, 3.191;7.839, 5.028, 6.112;χ^(2)=5.342, P<0.05).Conclusion:The treatment of hypertensive cerebral hemorrhage with stereotactic apparatus has a good effect,which is more conducive to hematoma drainage, reduces the compression damage to brain tissue, restores blood perfusion to brain tissue as early as possible, facilitates the recovery of injured nerves, and avoids the emergence of more serious complications at a later stage that affect the prognosis.
