摘　　要：目的:分析实时超声三维心动图(RT-3DE)诊断先天性心脏病(CHD)的临床价值。方法:选取2018年8月—2020年8月新乡市中心医院接收的74例疑似为CHD患者作为研究对象,所有患者均接受RT-3DE、心血管超声造影检查,以心血管超声造影结果为“金标准”,分析RT-3DE在先天性心脏病中的诊断价值。结果:经心血管超声造影检查显示,49例患者为阳性,25例患者为阴性。49例阳性患者中有23例为房间隔缺损,18例为室间隔缺损,8例为法洛四联症;RT-3DE诊断CHD的准确度为90.54%,灵敏度为91.84%,特异度为88.00%,阳性预测值为93.75%,阴性预测值为84.61%;RT-3DE诊断房间隔缺损、室间隔缺损、法洛四联症的符合率分别为95.65%、88.89%、87.50%,且经Kappa一致性检验显示,RT-3DE诊断CHD与金标准的一致性好。结论:RT-3DE对CHD具有较好的诊断价值,且对不同疾病类型均具有较高的符合率,可辅助临床鉴别疾病类型,从而为临床制定治疗方案提供有效参考。Objective:To analyze the clinical value of real-time ultrasound three-dimensional cardiography(RT-3DE) for the diagnosis of congenital heart disease(CHD).Methods:74 patients with suspected CHD admitted to the hospital from August 2018to August 2020 were selected for the study, and all patients underwent RT-3DE and cardiovascular ultrasonography. The diagnostic value of RT-3DE in congenital heart disease was analyzed using cardiovascular ultrasonography findings as the “gold standard”.Results:Cardiovascular ultrasonography showed positive results in 49 patients and negative results in 25 patients. Of the 49positive patients, 23 had atrial septal defects, 18 had ventricular septal defects, and 8 had tetralogy of Fallot. RT-3DE had an accuracy of 90.54%, sensitivity of 91.84%, specificity of 88.00%, positive predictive value of 93.75% and negative predictive value of84.61% for the diagnosis of CHD. The compliance rates of RT-3DE in diagnosing atrial septal defect, ventricular septal defect, and tetralogy of Fallot were 95.65%, 88.89%, and 87.50%, respectively. The Kappa concordance test showed that RT-3DE diagnosed CHD in good agreement with the gold standard.Conclusion:RT-3DE has a good diagnostic value for CHD and has a high compliance rate for different disease types, which can assist clinical identification of disease types and thus provide effective reference for clinical formulation of treatment plans.
