摘 要:目的 探究血清高迁移率族蛋白1(HMGB1)、肝素结合蛋白(HBP)、白细胞介素-10(IL-10)水平预测脓毒症患者预后的临床价值。方法 选取2018年3月-2021年3月天津市第五中心医院收治的92例脓毒症患者为研究组,另选取同期于医院进行健康体检者45名为对照组。检测两组HMGB1、HBP、IL-10、IL-6、C-反应蛋白(CRP)、降钙素原(PCT)水平,绘制受试者工作特征(ROC)曲线,分析血清HMGB1、HBP、IL-10水平对脓毒症患者预后的预测价值。结果 研究组血清HMGB1、HBP、IL-10、IL-6、CRP、PCT水平均高于对照组(P<0.05);脓毒血症患者血清HMGB1、HBP、IL-10水平与IL-6、CRP、PCT水平均呈正相关(P<0.05);住院15 d, 92例脓毒症患者死亡22例,生存70例,死亡组患者血清HMGB1、HBP、IL-10、IL-6、CRP、PCT水平均高于生存组(P<0.05);ROC曲线结果显示,血清HMGB1、HBP、IL-10联合检测预测脓毒症患者预后的曲线下面积(AUC)为0.897。结论 脓毒症患者血清HMGB1、HBP、IL-10呈高水平表达,且与患者预后有关,可作为该病预后预测的潜在指标。OBJECTIVE To explore the clinical value of serum high mobility group box 1(HMGB1), heparin-binding protein(HBP) and interleukin-10(IL-10) in prediction of prognosis of patients with sepsis. METHODS A total of 92 patients with sepsis who were treated in Tianjin Fifth Central Hospital from Mar 2018 to Mar 2021 were recruited as research subjects, meanwhile, 45 healthy people who received physical examination were chosen as the control group. The levels of HMGB1, HBP, IL-10, IL-6, C-reactive protein(CRP) and procalcitonin(PCT) were detected for the two groups. The values of serum HMGB1, HBP and IL-10 in prediction of prognosis of the patients with sepsis were analyzed by means of receiver operating characteristic(ROC) curves. RESULTS The levels of serum HMGB1, HBP, IL-10, IL-6, CRP and PCT of the study group were higher than those of the control group(P<0.05). The levels of serum HMGB1, HBP and IL-10 of the sepsis patients were positively correlated with the levels of IL-6, CRP and PCT(P<0.05). After 15 days of hospital stay, 22 of 92 sepsis patients died, and 70 survived. The levels of HMGB1, HBP, IL-10, IL-6, CRP and PCT of the death group were higher than those of the survival group(P<0.05). ROC curve analysis showed that the area under curve(AUC) of the joint detection of serum HMGB1, HBP and IL-10 was 0.897 in prediction of prognosis of the sepsis patients. CONCLUSION The sepsis patients show remarkable rise of serum HMGB1, HBP and IL-10. The detection of the three indexes may facilitate the prediction of prognosis and is worthy to be promoted.
