摘　　要：介绍陈日新教授“灸之要,气至而有效”的学术观点,并以其指导艾灸得气的临床应用。通过梳理古代文献关于得气的记载,结合灸法的临床研究成果,阐释“灸之要,气至而有效”学术观点,包括艾灸的核心是得气;艾灸得气是一组多维度的、与疗效密切相关的透热、扩热、传热等舒适的心身感应;艾灸得气能显著提高临床灸疗疗效;探敏得气、温控得气及手法得气是高效激发艾灸得气的关键;气至病所是艾灸得气技术操作的最高要求。临床中对不同病症选用不同得气穴位,如内脏病症常选用出现透热得气的穴位,经筋病症常选用出现非热觉得气的穴位,皮肤病症常选用出现扩热得气的穴位,远部取穴常选用出现传热得气的穴位。紧紧抓住艾灸要得气这个核心对于艾灸临床应用执简驭繁、提高疗效具有重要的指导价值。This paper introduced professor CHEN Rixin’s academic thought on“key point of moxibustion,arrival of qi produces curative effect”,as well as its clinical application.The records of Deqi in ancient documents were reviewed aligning with the clinical research achievements to further explain the viewpoint of“key point of moxibustion,arrival of qi produces curative effect”.It is considered that the core of moxibustion is Deqi.Deqi in moxibustion is a group of comfortable,multi-dimensional responses such as heat penetration,heat expanding and heat transmission related to curative effect.Deqi can significantly improve the clinical efficacy of moxibustion.It is pointed out that paying attention to selecting acupoint by sensation,regulating the temperature of moxibustion and emphasizing on manipulations are the key to effectively stimulate deqi in moxibustion.Qi arriving at the location of the disease is the highest requirement for the operation of deqi in moxibustion.In clinical practice,different acupoints are used for different diseases.For example,for visceral diseases,acupoints that obtain deqi through heat penetration are often used;meridian diseases usually take acupoints with non-thermal sensation,while skin diseases used acupoints and obtained deqi through heat expanding.Selection of distal points puts focus on heat transmission.This is the core of applying moxibustion in clinical practice,which deduces simplicity into complexity and is critical for improving clinical efficacy of moxibustion.
