摘 要:目的探究低嘌呤饮食结合高强度间歇训练应用于青年高尿酸血症痛风患者的效果及对肾功能、糖脂代谢指标的影响。方法选取我科2019年11月至2020年11月期间收治的114例青年高尿酸血症痛风患者为研究对象,根据数字表法随机分为对照组和观察组,两组均给予碳酸氢钠和非布司他药物控制血尿酸水平。对照组57例给予低嘌呤饮食,观察组57例在低嘌呤饮食基础上增加高强度间歇训练,对比两组患者糖代谢和脂代谢水平[空腹血糖(fasting plasma glucose,FPG)、总胆固醇(total cholesterol,TC)、总甘油三酯(total triglyceride,TG)]、胰岛素抵抗情况[空腹胰岛素(fasting insulin,FINS)、稳态模型胰岛素抵抗指数(homeostasis model insulin resistance index,HOMA-IR)]、肾功能[血尿酸(blood uric acid,BUA)、血清肌酐(serum creatinine,Scr)、血清尿素氮(blood urea nitrogen,BUN)]水平。结果治疗后观察组患者FPG、TC、TG、FINS、HOMA-IR、BUA、Scr和BUN水平均低于对照组患者(P<0.05)。结论采用高强度间歇训练结合低嘌呤饮食对青年高尿酸血症痛风患者进行干预可有效降低患者胰岛素抵抗现象,改善肾功能和糖脂代谢能力。Objective To explore the effect of low-purine diet combined with high-intensity interval training in young hyperuricemia gout patients and its influence on renal function and glucose and lipid metabolism indexes.Methods A total of 114 young hyperuricemia gout patients in our department from November 2019 to November 2020 were selected and grouped according to the random number table method.Both groups were given sodium bicarbonate and febuxostat to control blood uric acid levels.Fifty-seven patients in the control group were given a low-purine diet,and 57 patients in the observation group were given both low-purine diet and high-intensity interval training.The glucose and lipid metabolism levels,insulin resistance and renal function including fasting plasma glucose(FPG),total cholesterol(TC),total triglyceride(TG),fasting insulin(FINS),homeostasis model insulin resistance index(HOMA-IR),blood uric acid(BUA),serum creatinine(Scr)and blood urea nitrogen(BUN)levels of the two groups were compared.Results After the treatment,the levels of FPG,TC,TG,INS,HOMA-IR,BUA,Scr and BUN in the observation group were lower than those in the control group.Conclusion High-intensity interval training combined with low-purine diet intervention in young hyperuricemia gout patients can effectively reduce insulin resistance,and improve renal function and glucose and lipid metabolism.
