基于数据挖掘探析李伟兵治疗放射性直肠炎的用药规律及机制

摘　　要：目的 基于中医传承辅助平台V2.5软件挖掘李伟兵主任中医师治疗放射性直肠炎(Radiation proctopathy,RP)的用药规律,运用网络药理学方法研究核心组方治疗放射性直肠炎的机制。方法 筛选2016年7月至2021年4月就诊于南京医科大学附属肿瘤医院(江苏省肿瘤医院)李伟兵专家门诊的放射性直肠炎病例,将处方用药录入中医传承辅助平台(版本2.5),构建数据库,利用频次统计、关联规则等模块,归纳李伟兵治疗放射性直肠炎的组方规律,并运用网络药理学方法探索核心组方的有效成分、靶点及信号通路。结果 经数据清洗后共录入155首处方,经统计,共有中药145味,包含14味常用药物(频次>100次),20条常用药物组合(支持度为139),73对关联规则(置信度大于60%),核心组方由“黄芪-党参-半夏-黄芩-炮姜-炙甘草”组成,共含有117个有效成分,药物-疾病共同靶点129个,AKT1、VEGFA、HRAS等为关键靶点,PI3K-Akt等为关键信号通路。结论 李伟兵治疗放射性直肠炎注重攻补兼施、寒温并用、辛开苦降,核心组方可能通过PI3K-Akt等相关信号通路治疗放射性直肠炎。Objective To summarize and analyze the medication rule of LI Weibing’s treatment of radiation proctopathy(RP) based on V2.5 software of traditional Chinese medicine inheritance auxiliary platform, and to explore the mechanism of core prescription in the treatment of RP by network pharmacology. Methods We collected and screened cases with RP in LI Weibing’s outpatient clinic of the Affiliated Cancer Hospital of Nanjing Medical University(Jiangsu Cancer Hospital)from July 2016 to April 2021,recorded the prescription drugs one by one into the TCM inheritance auxiliary platform(Version 2.5)to build the database,and summarized the prescription rules of LI Weibing in the treatment of radioactive proctitis by using frequency statistics and association rules. The network pharmacology method was used to explore the effective components, targets and signal pathways of the core prescription. Results After data cleaning,a total of 155 prescriptions were recorded. According to statistics,there were 145 traditional Chinese medicines,including 14 commonly used drugs(frequency>100),20 commonly used drug combinations(support degree:139),and 73 pairs of association rules(confidence degree>60%). The core prescription was composed of "Astragali Radix-Codonopsis Radix-Pinelliae Rhizoma-Scutellariae Radix-Zingiberis Rhizoma Praeparatum-Glycyrrhizae Radix et Rhizoma Praeparata cum Melle",which contains 117 active ingredients,129 drug-disease common targets. It was found that AKT1,VEGFA,and HRAS were key targets. Besides,PI3K-Akt was key signaling pathways. Conclusion LI Weibing treats RP by the combination of tonification and purgation,frigotherapy and pyretotherapy,acrid medicinals open and bitter medicinals promote descent. The core prescription may play a role in the treatment of RP through PI3K-Akt and other related signaling pathways.
