喻清和治疗岭南地区小儿外感发热用药经验的数据挖掘

摘　　要：【目的】采用数据挖掘技术分析喻清和主任医师治疗岭南地区小儿外感发热的用药规律及特点。【方法】收集喻清和治疗小儿外感发热的有效病例的门诊处方,采用SPSS Modeler 18.0统计软件对纳入的处方中药进行频数分析、组方规律分析及药物关联规则分析。【结果】共纳入喻清和治疗小儿外感发热的门诊处方503首,涉及中药110味,其中使用频次>100次的中药有23味,使用频次居前8位的中药分别为甘草、北杏仁、辛夷、板蓝根、布渣叶、荆芥穗、青蒿、蝉蜕。纳入中药的功效分类主要涉及6大类,其中应用最多的3类药物分别为清热药、解表药、化痰止咳平喘药。使用频次较高的药物组合有蝉蜕-青蒿、蝉蜕-板蓝根、青蒿-板蓝根、荆芥穗-辛夷等。【结论】喻清和根据岭南地区小儿发热以外感风邪为主,随四时季节变化而兼挟寒、热、暑、湿、燥等邪气,且入里化热居多的致病特点,用药立足小儿肺和脾胃,以表里并治、清热时顾护阴液为治则,多选用清热及疏风解表药。同时基于岭南地区的地域与气候特点,选用化湿、渗湿之品,善用岭南地区特色中药。Objective To investigate chief physician YU Qing-He’s medication rules for treating pediatric exogenous fever in Lingnan area based on data mining technology. Methods The outpatient prescriptions for the effective cases of pediatric exogenous fever treated by chief physician YU Qing-He were collected, and SPSS Modeler 18 statistical software was used for frequency analysis, prescription pattern analysis and herbal association rule analysis of the included prescriptions of Chinese medicine. Results A total of 503 outpatient prescriptions for pediatric exogenous fever treated by chief physician YU Qing-He were included, involving110 Chinese medicines. Of the included herbs,there were 23 herbs with the medication frequency being more than100 times,and the top 8 Chinese medicines were Glycyrrhizae Radix,Armeniacae Amarum Semen,Magnoliae Flos, Isatidis Radix, Microcotis Folium, Schizonepetae Spica, Artemisiae Annuae Herba and Cicadae Periostracum. The classification of the actions of the included herbs mainly involved 6 major categories,of which the 3 most frequently-used categories were drugs for clearing heat,drugs for releasing the exterior and drugs for resolving phlegm,easing cough and relieving asthma. Herbal combinations with high medication frequency include Cicadae Periostracum-Artemisiae Annuae Herba, Cicadae Periostracum-Isatidis Radix, Artemisiae Annuae Herba-Isatidis Radix, Schizonepetae Spica-Magnoliae Flos, etc. Conclusion Pediatric fever in Lingnan area shows the characteristics of being mainly caused by externally-contracted wind pathogen and being complicated with pathogens of cold, heat, summer-heat, dampness and dryness in various seasons, and the exogenous pathogens are prone to transform into internal heat. Hence chief physician YU Qing-He adopts medicines for the treatment of pediatric fever in the Lingnan region based on regulating the children’s lungs and spleen-stomach,pays attention to the simultaneous treatment of the interior and exterior syndrome,follows the therapeutic principle of p
