陈兴华运用舌咽部火针点刺疗法治疗中风后吞咽障碍的临床经验

摘　　要：中风后吞咽障碍是脑卒中后常见的并发症之一,其临床疗效不尽人意。陈兴华教授认为,局部气血阻滞、枢机不利是引起中风后吞咽障碍的重要因素。治疗中风后吞咽障碍,陈兴华教授在“靳三针”中“舌三针”的治疗原理上,以中医的经络理论和西医的神经解剖生理机制为基础,结合火针的特性,采用舌咽部火针点刺疗法,该疗法的点刺部位为患者的舌根及咽后壁。采用火针疗法在舌咽部进行点刺,可改善局部气血流通,有效促进患者吞咽功能的恢复,从而改善中风后吞咽障碍症状。Post-stroke dysphagia is one of the common complications after stroke,and its clinical outcome is unsatisfactory. In the opinions of Professor CHEN Xing-Hua,local stagnation of qi and blood and dysfunction of pivot are the important factors causing post-stroke dysphagia. For the treatment of post-stroke dysphagia,Professor CHEN Xing-Hua put forward the method of fire-needle pricking in the glossopharyngeal region,which is applied to the root of tongue and the posterior pharyngeal wall. The glossopharyngeal fire-needle pricking method is originated from the tongue triple-needles of Jin’s three needle therapy and is based on the meridian theory of traditional Chinese medicine and the neuroanatomical and physiological mechanism of modern western medicine,which is also combined with the characteristics of fire-needling. It is believed that the use of fire-needle pricking method in the glossopharyngeal region can improve the local movement of qi and blood,effectively promote the recovery of the patients’ swallowing function,thereby relieving the symptoms of post-stroke dysphagia.
