基于数据挖掘的治疗支气管哮喘急性发作期与缓解期的方药规律对比分析

摘　　要：【目的】探讨中药方剂治疗支气管哮喘的急性发作期与缓解期的用药特点,为临床中医治疗支气管哮喘提供参考。【方法】检索并筛选中国知网、万方、维普等数据库中采用中药方剂治疗支气管哮喘急性发作期和缓解期的随机对照试验文献,将符合筛选标准的方药数据提取并录入至中医传承辅助平台(V2.5),并用平台集成的数据挖掘方法进行药物的频次统计、关联规则分析、新方挖掘。【结果】共筛选录入有效方剂238首,其中治疗支气管哮喘急性发作期的方剂162首,涉及142味中药;治疗缓解期的方剂76首,涉及123味中药。针对急性发作期的方剂药物中,用药频次居前的有麻黄、半夏、杏仁、甘草、紫苏子等;针对缓解期的方剂药物中,用药频次居前的有黄芪、甘草、茯苓、白术、五味子等。急性期常用的用药配伍模式为“半夏+麻黄”“麻黄+紫苏子”“麻黄+杏仁”等,挖掘得到新方组合9个;缓解期常用的用药配伍模式为“黄芪+甘草”“黄芪+白术”“黄芪+五味子”等,挖掘得到新方组合6个。【结论】中医药治疗支气管哮喘急性发作期以宣肺化痰平喘为主,兼用散寒、清热、解表清里等法;缓解期则以益气补肺为主,兼以宣肺化痰,并配合滋阴养血、健脾补肾之法。Objective To explore the medication rules of Chinese herbal formulas for the treatment of bronchial asthma during the acute stage and remission stage,and to provide reference for the clinical treatment of bronchial asthma with Chinese medicine. Methods Randomized controlled trials of Chinese herbal prescriptions for the treatment of acute stage and remission stage of bronchial asthma in databases such as CNKI,Wanfang Data and VIP were searched and screened. The prescriptions that met the screening criteria were extracted and the related data of the prescriptions were input to the Traditional Chinese Medicine Inheritance Support System(TCMISS)platform V2.5. And then the data mining methods provided by the platform were used to conduct frequency statistics, association rule analysis and mining of the new prescription. Results A total of 238 effective prescriptions were screened out, including 162 prescriptions of 142 herbal medicines for the acute stage of bronchial asthma, and 76 prescriptions of 123 herbal medicines for the remission stage of bronchial asthma.Among the herbs of the prescriptions for the acute stage,the frequently-used herbal medicines were Ephedrae Herba, Pinelliae Rhizoma, Armeniacae Amarum Semen, Glycyrrhizae Radix et Rhizoma, Perillae Fructus,etc. Among the herbs of the prescriptions for the remission stage,the most frequently-used ones were Astragali Radix, Glycyrrhizae Radix et Rhizoma, Poria, Atractylodis Macrocephalae Rhizoma, Schisandrae Chinensis Fructus, etc. For bronchial asthma at the acute stage, the common compatibility of medicines was “Pinelliae Rhizoma + Ephedrae Herba”“Ephedrae Herba + Perillae Fructus”“Ephedrae Herba + Armeniacae Amarum Semen”, and 9 new combinations were mined out. For bronchial asthma at remission stage, the common compatibility of medicines was“Astragali Radix + Glycyrrhizae Radix et Rhizoma”“Astragali Radix + Atractylodis Macrocephalae Rhizoma”“Astragali Radix + Schisandrae Chinensis Fructus”, and 6 new combinations were obtained.
