畸形精子症患者精子力学性质与体外受精结局的相关性研究

摘  要:目的观察畸形精子症患者的精子力学性质,分析其与体外受精结局的关系。方法选择2020年1月-2021年12月在甘肃省妇幼保健院行体外受精助孕的100例不育症患者,原子力显微镜检测精子的弹性模量,苯胺蓝染色检测精子染色质凝集水平。结果畸形精子症组患者精子的弹性模量显著小于精子形态正常组,差异具有统计学意义(P<0.05);染色质凝集异常水平高于精子形态正常组患者,但2组差异没有统计学意义(P=0.064);Logistic回归分析显示,畸形精子症组患者的精子弹性模量、染色质凝集水平均可能与体外受精助孕结局密切相关(P<0.05),两者的受试者操作特征曲线下面积分别为0.722、0.687,均有一定的预测价值。结论精子力学性质可以作为畸形精子症患者体外受精助孕结局的预测指标,精子弹性模量降低可能会增加患者体外受精助孕失败的发生风险。Objective To explore the mechanical properties of sperm in infertile patients with teratozoospermia,and analyze the relationship between them and the outcome of in vitro fertilization.Methods 100 patients diagnosed with infertility and who underwent in vitro fertilization in Gansu Provincial Maternity and Child-care Hospital from January 2020 to December 2021 were selected as research subjects.The modulus of elasticity was investigated via atomic force microscopy.Chromatin condensation was detected via aniline blue staining.Results Compared with the control group,the elasticity was significantly lower in teratozoospermia group(P<0.05),and the level of chromatin condensation was higher(P=0.064).Logistic regression analysis showed that both the elasticity and the chromatin condensation might be closely related to the outcome of intracytoplasmic sperm injection in patients with teratozoospermia(P<0.05).The area under the curve of the elasticity and chromatin condensation in predicting the outcome of in vitro fertilization in patients with teratozoospermia were 0.722 and 0.687,respectively,both having a certain predictive value.Conclusion The mechanical properties might be used as predictors of the outcome of in vitro fertilization in patients with teratospermia.The decrease of sperm elastic modulus may increase the failure in in vitro fertilization.
