阿司匹林缓冲片与阿司匹林肠溶片对胃肠道不良反应影响的系统评价

摘　　要：目的系统评价阿司匹林缓冲片与常规肠溶片阿司匹林相比哪种药胃肠道不良反应更轻。方法检索PubMed、The Cochrane Library、中国生物医学文献数据库、维普数据库、中国知识基础设施工程和万方数据库。检索时间从建库到2020年11月,收集低剂量长期阿司匹林缓冲片对比常规低剂量阿司匹林肠溶片治疗心脑血管疾病的研究。由2名研究员独立检索、筛选文献,提取资料并进行质量评价。采用RevMan 5.3软件进行Meta分析。结果最终纳入6篇文献,共计5392例患者,其中长期低剂量服用阿司匹林缓冲片(对照组)1159例,常规服用阿司匹林肠溶片(观察组)4233例。Meta分析结果显示:长期低剂量服用阿司匹林缓冲片和常规服用阿司匹林肠溶片相比,胃肠道溃疡发生率、糜烂发生率以及胃出血事件差异均无统计学意义(P>0.05)。结论在长期低剂量服用的状态下,阿司匹林肠溶片未表现出在胃肠道损伤方面比阿司匹林缓冲片更安全的优势。Objective To systematically evaluate the incidence of gastrointestinal adverse reactions of buffered aspirin compared with enteric-coated aspirin.Methods PubMed,The Cochrane Library,Chinese Biomedical Literature Database,VIP Data,China National Knowledge Infrastructure and Wanfang Data were searched from establishment of the databases to November 2020.The literature on long-term low dose buffered Aspirin versus low-dose enteric-coated Aspirin in the treatment of cardiovascular and cerebrovascular diseases was collected.Two researchers independently searched and screened the literature,extracted the data and evaluated the quality.RevMan 5.3 software was used for Meta-analysis.Results Six articles involving 5392 patients were included,including 1159 in the control group taking long-term low-dose buffered Aspirin and 4233 patients in the observation group taking low-dose enteric-coated Aspirin.Meta-analysis results showed that the incidence of gastrointestinal ulcers,erosions and gastric bleeding manifested no significant difference compared with enteric-coated Aspirin(P>0.05).Conclusions There is no significant difference between buffered Aspirin and enteric-coated Aspirin in the incidence of gastrointestinal erosions,ulcers and bleeding.
