摘 要:目的基于生物电阻抗分析法测量与识别多囊卵巢综合征(PCOS)患者多种亚型肥胖及体型特征,为其体重管理评估提供依据。方法应用人体成分分析仪测量1856例PCOS患者的体成分指标。结果从消瘦到肥胖,体质量指数与人体成分各指标呈正相关关系(P<0.05)。不同年龄组间正常体重、超重、肥胖以及体脂肪含量、体脂率、内脏脂肪等级等指标组间差异无统计学意义(P>0.05);18~24岁组与31~35岁组消瘦率以及肌肉量、去脂体重、骨骼肌量等指标差异均具有统计学意义(P<0.05)。体脂率联合体质量指数显示,正常体重肥胖型占55.7%,肌肉减少型肥胖占3.1%。四肢肌肉指数检出肌肉减少症者11.8%。结论PCOS患者中普遍存在体脂肪和(或)肌肉量失衡,识别正常体重肥胖型、肌肉减少型肥胖、肌肉减少症等体型,给予体重管理方案将极具临床价值。Objective To measure and identify the characteristics of obesity and body shape in different subtypes of polycystic ovary syndrome(PCOS)patients based on bioelectrical impedance analysis,and to provide a basis for weight management evaluation.Methods A total of 1856 patients with PCOS were selected and their body composition characteristics measured by a body composition analyzer.Results From emaciation to obesity,body mass index(BMI)was positively correlated with the body composition(P<0.05).There were no significant difference in the distribution of normal weight,overweight,obesity,body fat mass,body fat percentage and visceral fat grade among different age groups(P>0.05),while significant differences in the emaciation rate,muscle mass,fat-free body weight and skeletal muscle mass appeared between 18~24 years old group and 31~35 years old group(P<0.05).Body fat percentage combined with BMI showed that the proportion of normal weight obesity was 55.7%,and the proportion of sarcopenic obesity was 3.1%.Sarcopenia was detected in 11.8%of appendicular skeletal muscle mass index.Conclusions Body fat and/or muscle mass imbalance are common in PCOS patients.Early identification of normal weight obesity,sarcopenia obesity,sarcopenia and other subtypes,and different weight management programs will be of great clinical value.
