摘　　要：目的:探讨铁蛋白、钙、磷、甲状旁腺激素(PTH)水平与维持性血液透析(MHD)患者预后的关系。方法:回顾性分析130例MHD患者临床资料,检测外周血铁蛋白、血红蛋白、白蛋白、肌酐、尿酸、钙、磷、PTH、C-反应蛋白(CRP)水平。保持随访至2021年5月,记录患者生存情况,并对影响MHD患者预后的因素进行Cox回归分析。结果:130例MHD患者死亡51例,存活79例。生存组年龄、透析时间、CRP、铁蛋白、肌酐、尿酸、磷、PTH水平低于死亡组,钙水平高于死亡组(均P<0.05)。不同铁蛋白、钙、磷、PTH水平患者5年生存率比较差异有统计学意义(均P<0.05)。高龄、高铁蛋白、低钙、高磷、高PTH、高CRP水平是MHD患者预后的独立危险因素(均P<0.05)。结论:血清铁蛋白、钙、磷、PTH水平与MHD患者预后密切相关,是影响MHD患者预后的独立危险因素。Objective:To explore the relationship between ferritin,calcium,phosphorus,parathyroid hormone(PTH)levels and the prognosis of maintenance hemodialysis(MHD)patients.Methods:The clinical data of 130 MHD patients were retrospectively analyzed.The levels of ferritin,hemoglobin,albumin,creatinine,uric acid,calcium,phosphorus,PTH,and CRP in peripheral blood were detected.The patients were followed up until May 2021,and the survival of patients was recorded.Cox regression analysis was used to analyze the factors affecting the prognosis of MHD patients.Results:Among 130 MHD patients,51 patients died and 79 patients survived.The age,dialysis time,CRP,ferritin,creatinine,uric acid,phosphorus and PTH levels of the survival group were lower than those of the death group,and the calcium level was higher than that of the death group(all P<0.05).There were significant differences in 5-year survival rates among patients with different levels of ferritin,calcium,phosphorus and PTH(all P<0.05).Age,high ferritin,low calcium,high phosphorus,high PTH and high CRP levels were independent risk factors for the prognosis of MHD patients(all P<0.05).Conclusion:Serum ferritin,calcium,phosphorus and PTH levels are closely related to the prognosis of MHD patients,and they are independent risk factors for the prognosis of MHD patients.
