摘 要:目的:探究过敏性紫癜患儿外周血微小RNA-155(miR-155)、Toll样受体2(TLR2)、TLR4、氧化应激指标表达水平与辅助性T细胞17(Th17)/调节性T细胞(Treg)平衡的关系。方法:纳入过敏性紫癜患儿102例为观察组,另选择同期体检健康儿童120例为对照组。比较两组外周血miR-155、TLR2、TLR4表达水平,氧化应激指标[晚期氧化蛋白产物(AOPP)、丙二醛(MDA)和超氧化物歧化酶(SOD)]水平,以及Th17/Treg平衡。分析外周血miR-155、TLR2、TLR4、氧化应激指标表达水平与Th17/Treg平衡的相关性。结果:观察组外周血miR-155、TLR2、TLR4、AOPP、MDA、Th17、Th17/Treg水平高于对照组,Treg和SOD水平低于对照组(均P<0.05)。Pearson相关性分析结果显示,miR-155、TLR2、TLR4、AOPP、MDA与Th17呈正相关,SOD与Th17呈负相关(均P<0.05);miR-155、TLR2、TLR4、AOPP、MDA与Treg呈负相关,SOD与Treg呈正相关(均P<0.05);miR-155、TLR2、TLR4、AOPP、MDA与Th17/Treg呈正相关,SOD与Th17/Treg呈负相关(均P<0.05)。结论:过敏性紫癜患儿外周血miR-155、TLR2、TLR4表达水平升高,氧化应激水平增加,且存在Th17/Treg失衡。外周血miR-155、TLR2、TLR4、氧化应激指标表达水平与Th17/Treg失衡具有相关性,可能为疾病诊治提供新的依据。Objective:To investigate the relationship between the peripheral blood expression levels of microRNA-155(miR-155),Toll-like receptoR2(TLR2),TLR4,oxidative stress indicators and T helper cell 17(Th17)/regulatory T cell(Treg)balance in peripheral blood of children with anaphylactoid purpura.Methods:A total of 102 children with anaphylactoid purpura were selected as the observation group,and 120 healthy children with physical examination during the same period were selected as the control group.The expression levels of miR-155,TLR2,TLR4,oxidative stress indicators(AOPP,MDA and SOD)and Th17/Treg balance in peripheral blood in the two groups were compared.The correlation between the expression levels of miR-155,TLR2,TLR4,oxidative stress indicators and Th17/Treg balance in peripheral blood was analyzed.Results:Compared with the control group,the observation group had higher levels of miR-155,TLR2,TLR4,AOPP,MDA,Th17 and Th17/Treg in peripheral blood,and lower levels of Treg and SOD(all P<0.05).Pearson correlation analysis showed that miR-155,TLR2,TLR4,AOPP and MDA were positively correlated with Th17,and SOD was negatively correlated with Th17(all P<0.05);miR-155,TLR2,TLR4,AOPP and MDA were negatively correlated with Treg,and SOD was positively correlated with Treg(all P<0.05);miR-155,TLR2,TLR4,AOPP and MDA were positively correlated with Th17/Treg,and SOD was negatively correlated with Th17/Treg(all P<0.05).Conclusion:Children with anaphylactoid purpura have increased expression levels of miR-155,TLR2 and TLR4 in peripheral blood,an increased level of oxidative stress,and an imbalance of Th17/Treg.The expression levels of miR-155,TLR2,TLR4 and oxidative stress in peripheral blood are correlated with Th17/Treg imbalance,which may provide a new basis for diagnosis and treatment of diseases.
