摘 要:目的:探讨鼻气道炎症(NAI)和鼻气道阻塞(NAO)对混合牙列期儿童颌面发育的影响。方法:收集以鼻塞、张口呼吸及睡时打鼾为主诉的混合牙列期患儿79例,根据电子鼻咽镜检查结果分为NAO组(37例)和NAI组(42例)。对所有入组患儿拍摄自然头位、闭唇、上下牙列自然咬合位、吸气期标准的头颅侧位片。使用口腔科Onxy ceph头影测量软件,选取10个标志点进行4个角度[下颌平面角(ML-FH)、SNB角、ANB角、下颌角(Ar-Go-Me)]和1条线距[前下面高(ANS-Me)]的测量。比较两组患儿与正常人群(正常组)颌面发育指标,以评判NAO或NAI是否会造成颌面发育的异常,正常组相关指标参考值摘自《口腔正畸学》第7版。比较NAO与NAI患儿颌面发育指标,以评价NAO与NAI所造成的颌面发育异常是否存在差异。结果:NAO组ML-FH和ANS-Me高于正常组(均P<0.05),但两组SNB角、ANB角、Ar-Go-Me比较差异无统计学意义(均P>0.05)。NAI组ML-FH和ANS-Me高于正常组(均P<0.05),但两组SNB角、ANB角、Ar-Go-Me比较差异无统计学意义(均P>0.05)。NAO组与NAI组ML-FH、ANS-Me比较差异无统计学意义(均P>0.05)。结论:NAO和NAI均会造成患儿颌面发育异常,下颌平面角增大,前下面高度变大,但NAO和NAI所造成的颌面发育异常无明显差异。Objective:To investigate the effects of nasal airway inflammation(NAI)and nasal airway obstruction(NAO)on maxillofacial development in children with mixed dentition.Methods:Seventy-nine children with mixed dentition who had the chief complaints of nasal obstruction,mouth breathing,and snoring during sleep were enrolled.According to the results of electronic nasopharyngoscopy,they were divided into NAO group(37 cases)and NAI group(42 cases).Lateral cephalograms were taken in the natural head position,lip closure,upper and lower dentition natural occlusion position,and standard inspiratory phase.Using Onxy ceph cephalometric software,10 landmarks were selected for measurement of 4 angles(ML-FH,SNB angle,ANB angle,Ar-Go-Me)and 1 line distance(ANS-Me).The maxillofacial development indexes of the two groups were compared with those of the normal population(normal group)to determine whether NAO or NAI could cause abnormal maxillofacial development.The reference values of the normal group were extracted from the 7th edition of“Orthodontics”.The maxillofacial developmental parameters of children with NAO and NAI were compared to evaluate the differences in maxillofacial developmental abnormalities caused by NAO and NAI.Results:ML-FH and ANS-Me in the NAO group were higher than those in the normal group(all P<0.05),but there were no significant differences in SNB angle,ANB angle and Ar-Go-Me between the two groups(all P>0.05).The ML-FH and ANS-Me in the NAI group were higher than those in the normal group(all P<0.05),but there were no significant differences in SNB angle,ANB angle and Ar-Go-Me between the two groups(all P>0.05).There was no significant difference in ML-FH and ANS-Me between NAO group and NAI group(all P>0.05).Conclusion:Both NAO and NAI can cause maxillofacial abnormalities,including the increase of the mandibular plane angle and the increase of the height of the anterior lower face,but there is no significant difference between NAO and NAI.
