摘 要:目的:探讨生物钟基因CLOCK及褪黑素受体MT1和MT2基因与2型糖尿病(T2DM)患者肠道菌群及炎症因子的相关性。方法:选择T2DM患者80例为观察组,选择同期进行健康体检者80例为对照组。检测两组血清CLOCK、MT1和MT2基因水平,以及超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)和IL-18水平。检测两组粪便中肠杆菌、双歧杆菌、乳杆菌、肠球菌、双歧杆菌/肠杆菌丰度。分析观察组血清CLOCK、MT1和MT2基因水平与T2DM患者肠道菌群及炎症因子的相关性。结果:观察组血清CLOCK、MT1、MT2基因水平明显较对照组低(均P<0.05)。观察组肠杆菌水平明显较对照组高,双歧杆菌、乳杆菌、双歧杆菌/肠杆菌水平明显较对照组低(均P<0.05)。观察组hs-CRP、IL-6、TNF-α、IL-18水平明显较对照组高(均P<0.05)。观察组血清CLOCK、MT1、MT2基因水平与肠杆菌、hs-CRP、IL-6、TNF-α、IL-18水平呈负相关,与双歧杆菌、乳杆菌、双歧杆菌/肠杆菌水平呈正相关(均P<0.05)。结论:血清生物钟相关基因CLOCK、MT1和MT2基因与T2DM患者肠道菌群及炎症因子均有一定相关性,可作为潜在的诊断指标及治疗靶点。Objective:To investigate the correlation of clock-related gene CLOCK,melatonin receptor MT1 and MT2 genes with intestinal microbiota and inflammatory factors in patients with type 2 diabetes mellitus(T2DM).Methods:Eighty T2DM patients were selected as observation group,and Eighty healthy people for physical examination during the same period were selected as control group.The levels of CLOCK,MT1 and MTT2,hs-CRP,IL-6,TNF-αand IL-18 in serum of the two groups were detected.The abundance of enterobacterium,bifidobacterium,lactobacillus,enterococcus and bifidobacterium/enterobacterium in feces of the two groups were detected.The correlation of CLOCK,MT1 and MT2 genes with intestinal microbiota and inflammatory factors in T2DM patients was analyzed.Results:The levels of CLOCK,MT1 and MT2 genes in the observation group were significantly lower than those in the control group(all P<0.05).The level of enterobacter in the observation group was significantly higher than that in the control group,and the levels of bifidobacterium,lactobacillus and bifidobacterium/enterobacter in the observation group were significantly lower than those in the control group(all P<0.05).The levels of hs-CRP,IL-6,TNF-αand IL-18 in the observation group were significantly higher than those in the control group(all P<0.05).The levels of CLOCK,MT1 and MT2 genes in the observation group were negatively correlated with the levels of enterobacterium,hs-CRP,IL-6,TNF-αand IL-18,and positively correlated with the levels of bifidobacterium,lactobacillus and bifidobacterium/enterobacterium(all P<0.05).Conclusion:The serum clock-related genes CLOCK,MT1 and MT2 have certain correlation with intestinal microbiota and inflammatory factors in T2DM patients,which can be used as potential diagnostic indicators and therapeutic targets.
