中药熏蒸治疗腰椎内固定术后腰痛血瘀证疗效研究

摘　　要：目的:观察中药熏蒸疗法治疗腰椎后路椎板减压椎弓根钉内固定术后腰痛的临床疗效。方法:选取腰椎后路椎板减压椎弓根钉内固定术后腰痛患者66例,并随机分为两组,每组33例。治疗组采用中药熏蒸疗法治疗,对照组予以常规西药内服治疗,在首次治疗前、治疗3个疗程后分别记录和观察患者视觉疼痛模拟评分(VAS)、Roland-Morris(RMDQ)腰椎功能评分、Oswestry功能障碍指数(ODI)评价疗效、腰椎活动曲度及总有效率。结果:3个疗程治疗后,治疗组与对照组VAS、RMDQ腰椎功能评分、ODI下降,差异有统计学意义(均P<0.05)。治疗后,治疗组VAS、RMDQ与ODI评分均低于对照组,差异有统计学意义(均P<0.05)。治疗后腰椎活动曲度较治疗前增大,且治疗组的曲度大于对照组,差异有统计学意义(P<0.05)。治疗组3个疗程后的总有效率为96.97%,对照组总有效率为84.85%,组间比较,差异有统计学意义(P<0.05)。结论:中药熏蒸法治疗内固定术后腰痛疗效较好,可有效改善患者预后情况。Objective:To observe clinical efficacy of traditional Chinese medicine fumigation in the treatment of back pain after posterior lumbar lamina decompression and pedicle screw internal fixation.Methods:66 patients with back pain after posterior lumbar decompression and pedicle screw internal fixation were selected and randomly divided into two groups,33 patients in each group.The treatment group were treated with traditional Chinese medicine fumigation therapy,and the control group were treated with conventional western medicine.Visual pain analogue scale(VAS),roland-morris(RMDQ)lumbar function score and oswestry disability index(ODI)were recorded before the first treatment and afteR3 courses of treatment.Results:AfteR3 courses of treatment,VAS,RMDQ and ODI scores in the treatment group and the control group were significantly decreased,difference statistically significant(all P<0.05).After treatment,VAS,RMDQ and ODI scores in the treatment group were lower than those in the control group,difference statistically significant(all P<0.05).After treatment,curvature of lumbar movement increased and the curvature in the treatment group was greater than that in the control group,difference statistically significant(P<0.05).AfteR3 courses of treatment,total effective rate in the treatment group 96.97%higher than that in the control group 84.85%,difference statistically significant(P<0.05).Conclusion:Traditional Chinese medicine fumigation has definite effect on the treatment of back pain after internal fixation,and can significantly improve the clinical prognosis of patients.
