太极悬灸联合祛湿强骨汤治疗膝骨性关节炎疗效研究

摘　　要：目的:探讨太极悬灸联合祛湿强骨汤治疗膝骨性关节炎临床疗效。方法:选择膝骨性关节炎患者86例,根据随机数字表法分为对照组与观察组,各43例,对照组给予祛湿强骨汤治疗,观察组在对照组基础上给予太极悬灸治疗。观察两组治疗前后临床疗效、中医证候评分、WOMAC量表评分以及实验室指标[白细胞介素-1(IL-1)、基质金属蛋白酶-13(MMP-13)、基质金属蛋白酶-3(MMP-3)],超声指标[内侧外缘关节间隙(PJS)、内侧半月板突出值(MRD)]。结果:治疗后观察组总有效率为92.86%高于对照组的76.19%,差异有统计学意义(P<0.05)。治疗后两组中医证候评分、WOMAC量表评分、血清IL-1、MMP-13、MMP-3水平均较治疗前降低,且观察组低于对照组,差异有统计学意义(均P<0.05)。治疗后两组PJS值较治疗前升高,MRD值较治疗前下降,且观察组改变程度大于对照组,差异有统计学意义(均P<0.05)。结论:太极悬灸联合祛湿强骨汤治疗膝骨性关节炎疗效较好,能够有效改善炎症反应和骨关节状态。Objective:To explore the clinical observation of Taiji moxibustion combined with Qushi Qianggu decoction in treatment of knee osteoarthritis.Methods:86 patients with knee osteoarthritis were selected and divided into control group and observe group,according to random number table method.The clinical efficacy,TCM syndrome scores,WOMAC scale score,laboratory indexes[interleukin-1(IL-1),metalloproteinase-13(MMP-13),matrix metalloproteinase-3(MMP-3)],ultrasound indicators[medial-lateral joint space(PJS),medial meniscus value(MRD)]were observed before and after treatment in the two groups.Results:After treatment,the total effective rate in the observation group was 92.86%higher than that in the control group 76.19%,difference statistically significant(P<0.05).TCM syndrome scores,WOMAC scale score,serum IL-1,MMP-13,MMP-3 levels were lower than pre-treatment,and the observe group were lower than those in the control group,difference statistically significant(all P<0.05).After treatment,PJS values were higher than pre-treatment,and the MRD values lower than pre-treatment,degree of change in the observation group greater than those in the control group,difference statistically significant(all P<0.05).Conclusion:Taiji moxibustion combined with Qushi Qianggu decoction has significant clinical effect in the treatment of knee osteoarthritis.It can effectively improve inflammatory response and bone and joint condition.
