摘  要:目的 探讨纯母乳喂养母乳乳汁中功能性低聚糖水平与迟发型母乳性黄疸(LBMJ)的相关性,为LBMJ的病因研究提供理论依据。方法 选取昆明医科大学第一附属医院儿科门诊和预防保健科0~3月龄纯母乳喂养足月儿60例,根据不同排便频次分为A、B及C组,即3 d以内排便1次或排便≤3次/d为A组;排便>3次/d为B组;超过4 d排便1次为C组;采用手挤奶方法收集婴儿母亲乳汁,收集后立即存置于-80℃冰箱内,待用超高效液相色谱质谱仪检测功能性低聚糖的含量。结果行统计学分析。结果 (1)纯母乳喂养母乳乳汁中3种功能性低聚糖水平在婴儿不同排便频次组比较,差异有统计学意义(H2-FL=0.978,P2-FL=0.013;H三糖=13.991,P三糖=0.001;H四糖=1.505,P四糖=0.041);(2)不同排便频次组黄疸与非黄疸婴儿比较,差异有统计学意义[χ^(2)(A)=4.472,P(A)=0.001;χ^(2)(B)=5.196,P(B)=0.001;χ^(2)(C)=3.000,P(C)=0.017];(3)不同功能性低聚糖在婴儿黄疸及非黄疸组差异无统计学意义(H2-FL=0.459,P2-FL=0.646;H三糖=0.988,P三糖=0.323;H四糖=0.633,P四糖=0.527),但黄疸组中3种功能性低聚糖水平较非黄疸组高。结论 0~3月龄纯母乳喂养LBMJ婴儿,母乳中低聚糖水平较非LBMJ婴儿高,低聚糖水平越高,排便频次越多。Objective To analyze the relationship between functional oligosaccharide level and late-onset breast-milk jaundice(LBMJ) during exclusive breastfeeding, and to provide theoretical basis for the study of LBMJ etiology.Methods Sixty full-term infants aged 0-3 months were selected from our hospital.According to different defecation frequency, they were divided into group A,B and C,namely, one defecation within 3 days or less than 3 defecation per day was group A;Defecation > 3 times/day was group B;One defecation over 4 days was group C;The mother’s milk was collected by hand milking, and immediately stored in a refrigerator at-80 ℃.The content of functional oligosaccharides was determined by ULTRA performance liquid chromatography mass spectrometer.The results were statistically analyzed.Results(1) Comparison of the levels of three functional oligosaccharides in breast milk of infants with different defecation frequency groups showed statistically significant differences in exclusive breastfeeding(H2-FL=0.978,P2-FL=0.013;Htrisaccharide=13.991,Ptrisaccharide=0.001;Htetrose=1.505,Ptetrose=0.041);(2)The difference between jaundice and non-jaundice infants in different defecation frequency group was statistically significant[χ^(2)(A)=4.472,P(A)=0.001;χ^(2)(B)=5.196,P(B)=0.001;χ^(2)(C)=3.000,P(C)=0.017];(3)Different functional oligosaccharides showed no statistical difference between the jaundice group and the non-jaundice group(H2-FL=0.459,P2-FL=0.646;Htrisaccharide=0.988,Ptrisaccharide=0.323;Htetrose=0.633,Ptetrose=0.527),but the levels of three functional oligosaccharides in the jaundice group were higher than those in the non-jaundice group.Conclusion In LBMJ infants aged 0-3 months, the level of oligosaccharide in breast milk was higher than that in non-LBMJ infants.The higher the level of oligosaccharide, the more frequent the defecation.
