刺血疗法联合苗药痛风酊对急性痛风关节炎模型大鼠IL-1β、IL-18、TNF-α及滑膜功能的影响

摘　　要：目的:探讨刺血疗法联合苗药痛风酊对急性痛风性关节炎模型大鼠血清炎性因子IL-1β、IL-18、TNF-α及滑膜功能的影响。方法:将36只健康SD大鼠随机分为空白对照组(9只)和模型制备组(27只)两组。空白组不进行处理;对于模型制备组,除进行造模处理外,还在右后肢踝关节关节腔内注射尿酸钠(MSU)溶液,制备急性痛风性关节炎模型,然后将造模成功后的模型制备组随机分为3组,分别为模型对照组、阳性对照组和治疗组,每组9只。其中阳性对照组给予秋水仙碱灌胃治疗,模型组给予等容积量的蒸馏水灌胃,治疗组给予刺血疗法联合苗药痛风酊外擦。观察造模后1~7天关节肿胀度;造模7天后采用苏木精-伊红染色观察滑膜病理改变,ELISA检测血清中IL-1β、IL-18、TNF-α水平。结果:(1)模型对照组大鼠关节肿胀度,血清中IL-1β、IL-18、TNF-α水平升高,滑膜组织有明显炎性细胞浸润和滑膜细胞肿胀、增生;(2)阳性对照组、治疗组关节肿胀度,血清中IL-1β、IL-18、TNF-α水平明显改善。结论:刺血疗法联合苗药痛风酊外擦能明显改善急性痛风性关节炎大鼠关节症状,下调炎症因子表达水平,改善滑膜功能。Objective:To investigate the effects of blood pricking therapy combined with Miao gout tincture on serum inflammatory factors IL-1β,IL-18,TNF-α and synovial function in rats with acute gouty arthritis model.Methods:The 36 rats were randomly divided into 2 groups, the blank control group(9 rats) and the model preparation group(27 rats),and the acute gouty arthritis model was prepared by intra-articular injection of sodium urate(MSU) solution into the right hind limb ankle joint, and then the model preparation group was randomLy divided into 3 groups, the model control group, the positive control group and the treatment group, respectively.9 animals in each group.The blank group was not treated, the positive control group was treated with colchicine gavage, the model group was treated with distilled water gavage in equal volume, and the treatment group was treated with blood pricking therapy combined with external rubbing of Miao gout tincture.The degree of joint swelling was observed 1-7 days after modeling;hematoxylin-eosin staining was used to observe synovial pathological changes 7 days after modeling, and serum levels ofinterleukin 1β,interleukin 18 and tumor necrosis factor α were detected by ELISA.Results:The degree of joint swelling, serum levels of interleukin 1β,interleukin 18 and tumor necrosis factor α were increased in the model control group rats, and synovial tissue had obvious inflammatory cell infiltration and synovial cell proliferation.Positive control group and treatment group showed significant improvement in joint swelling, serum levels of interleukin 1β,interleukin 18 and tumor necrosis factor α.Conclusion:The combination of blood pricking therapy and external rubbing of Miao gout tincture can significantly improve the joint symptoms, down-regulate the expression level of inflammatory factors and improve the synovial membrane function in rats with acute gouty arthritis.
