摘 要:目的:探讨脑脊液引流(CSFD)联合针刺治疗急性主动脉夹层(AAD)术后并发截瘫的疗效。方法:选取AAD术后出现截瘫的患者共7例,其中女性2例、男性5例,年龄(54.1±4.1)岁。患者给予早期CSFD术联合针刺治疗,观察分析其治疗效果。结果:早期行CSFD术联合针刺治疗后,7例截瘫患者的SCIMⅢ评分明显高于治疗前(P<0.05),术后随访时间(15.5±8.8)个月,随访期间7例患者均病情平稳,血压控制良好;其中2例截瘫,1例轻瘫,4例日常生活活动能力、肌力大致恢复正常,总有效率71.4%。结论:截瘫是急性主动脉夹层术后的严重并发症,早期采取脑脊液引流联合针刺治疗,可以促进患者的肢体功能恢复,提高患者的近中期生存质量,值得临床进一步推广应用。Objective:To investigate the curative effect of cerebrospinal fluid drainage(CSFD) combined with acupuncture in the treatment of acute aortic dissection(AAD) complicated with paraplegia.Methods:7 patients with paraplegia after AAD,aged(54.1±4.1) years, After early CSFD cooperate during acupuncture treatment, observe and analyze the treatment effect.Results:SCIM Ⅲ scores of 7 patients were significantly higher than before treatment after early CSFD combined with acupuncture(P<0.05).Postoperative follow up time(15.5±8.8) months.During the follow up, all the 7 patients were in stable condition with good blood pressure control.Among them, 2 cases were paraplegia, 1 case was paresis, and 4 cases were normal in daily activities and muscle strength.The total effective rate 71.4%.Conclusion:Paraplegia is a serious complication after AAD.Early intervention of CSFD and acupuncture can promote the recovery of limb function and improve the quality of life of patients in the near and middle period, which is worthy of further clinical research and promotion.
