摘　　要：快速眼球运动睡眠期行为障碍(RBD)是指在视频多导睡眠监测(v-PSG)上监测到快速眼球运动睡眠期异常行为发作或肌肉失迟缓现象(RWA),其中特发性RBD是α-突触核病等神经变性疾病的前驱期标志,目前临床上西医药物疗效欠佳,并且有一定副作用。RBD暂无特定的中医病名,但“梦魇”“梦呓”“梦惊”等均为其特征性的临床表现。“肝脾失调”是贯穿该病发生发展始终的核心病机,气机升降失司、血液功能失常、风痰上扰为三大主要病因病机,以疏肝理气、养血活血、熄风化痰法拓展RBD新的中医辨证思路,随证灵活化裁,以期为中医药治疗睡眠疾病提供更多的理论基础及临床经验,为未来临床及科研提供借鉴和启发。Rapid-eye-movement sleep behavior disorder(RBD) is a common parasomnia observed by video-polysomnography(v-PSG).It is usually characterised as either abnormal behaviours seen in REM sleep or REM sleep with-out atonia(RWA).Moreover, idiopathic RBD has become a predictor of neurodegenerative diseases, especially in α-synucleinaggregation disease.Due to their problematic side effects and insufficient treatment in current clinical practice, RBD has no specific name in the traditional Chinese medicine system.However, it can be named as “Meng Yan”,“Meng Yi” and “Ye Jing” based on its characteristic clinical manifestations.The theory of “disharmony of liver-spleen” is a core pathogenesis throughout the occurrence and development of RBD.The three main etiology and pathogenesis of RBD lies in the abnormal rise and fall of Qi, the dysfunction of blood, and the upward attack of wind-phlegm.it is expanded to soothing the liver and regulating Qi, nourishing and invigorating blood, as well as extinguishing wind and removing phlegm to provides a new way of thinking for treatment for RBD based on syndrome differentiation.Meanwhile, it is applied with the consideration of different symptoms during the treatment.This paper is expected to provide a different theoretical basis and clinical experience of the TCM treatment for the sleep disorder, aiming to provide some recommendations and inspirations for the future clinical practice and fundamental research on RBD treatment.
