摘　　要：目的探讨侧卧位手术患者术中急性压疮形成的危险因素。方法回顾性分析2021年3月至2022年3月于南昌大学第四附属医院行侧卧位手术治疗的118例患者的临床资料,根据是否发生急性压疮将患者分为发生组和未发生组,统计两组患者的基础资料,采用多因素Logistic回归分析探讨侧卧位手术患者发生术中急性压疮的危险因素。结果118例侧卧位手术患者中,13例患者发生术中急性压疮,发生率11.02%。单因素分析结果显示,发生组患者的手术时间≥2h、体质量指数(body mass index,BMI)≥25kg/m^(2)、术前血红蛋白<60g/L、术中无保护措施、术中体温<36℃占比均显著高于未发生组(P<0.05);多因素Logistic回归分析结果显示,手术时间≥2h、BMI≥25kg/m^(2)、术前血红蛋白<60g/L、术中无保护措施、术中体温<36℃均是侧卧位手术患者发生术中急性压疮的独立危险因素(P<0.05)。结论侧卧位手术患者术中易发生急性压疮,手术时间≥2h、BMI≥25kg/m^(2)、术前血红蛋白<60g/L、术中无保护措施、术中体温<36℃均是其独立危险因素,临床可针对高危因素制定干预措施,以降低压疮发生率。Objective To explore the risk factors of acute pressure ulcers in patients undergoing lateral decubitus operation.Methods The data of 118 patients who underwent lateral decubitus surgery in the Fourth Affiliated Hospital of Nanchang University from March 2021 to March 2022 were retrospectively analyzed.According to the occurrence of acute pressure ulcers,the patients were divided into the occurrence group and the non-occurrence group.The basic data of the two groups were analyzed,and the risk factors of the occurrence of intraoperative acute pressure ulcers in patients undergoing lateral decubitus surgery were discussed by multivariate Logistic regression analysis.Results Among 118 patients undergoing lateral decubitus surgery,13 patients developed acute pressure ulcers during operation,and the incidence was 11.02%.Univariate analysis showed that the proportion of operation time≥2h,body mass index(BMI)≥25kg/m^(2),preoperative hemoglobin<60g/L,no intraoperative protection measures,and intraoperative body temperature<36℃in occurrence group were higher than those in non-occurrence group(P<0.05).Multivariate Logistic regression analysis showed that operation time≥2h,BMI≥25kg/m^(2),preoperative hemoglobin<60g/L,no intraoperative protective measures,intraoperative body temperature<36℃were independent risk factors of intraoperative acute pressure ulcer in patients undergoing lateral recumbency surgery(P<0.05).Conclusion Patients undergoing lateral decubitus surgery are prone to acute pressure ulcers.Operation time≥2h,BMI≥25kg/m^(2),preoperative hemoglobin<60g/L,no intraoperative protection measures,and intraoperative body temperature<36℃are independent risk factors.Clinical interventions can be formulated for high-risk factors to reduce the incidence of pressure ulcers.
