带锁髓内钉固定对胫骨下段骨折的治疗效果及对骨折愈合情况的影响

摘　　要：目的探讨微创经皮钢板内固定与带锁髓内钉固定对胫骨下段骨折患者的临床疗效。方法选取2018年6月至2019年12月广州市第一人民医院收治的67例胫骨下段骨折患者为研究对象,采用随机数字表法分为对照组(33例)和观察组(34例)。对照组接受微创经皮钢板内固定治疗,观察组接受带锁髓内钉固定治疗。比较两组患者的手术指标,随访18个月的骨折愈合效果和并发症发生情况。结果观察组的手术时间及术后下地时间短于对照组,术中出血量少于对照组,手术切口长度短于对照组,差异有统计学意义(P<0.05)。随访18个月,观察组患者的骨折愈合优良率高于对照组,并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论与微创经皮钢板内固定相比,带锁髓内钉固定治疗胫骨下段骨折患者效果更佳,手术效果更好,患者恢复更快,且并发症较少。Objective To explore the clinical efficacy of minimally invasive percutaneous plate fixation and interlocking intramedullary nail fixation in the treatment of lower tibial fractures.Methods Sixty-seven patients with lower tibial fracture admitted to Guangzhou First People's Hospital from June 2018 to December 2019 were selected as the research objects,and they were divided into control group(33 cases)and observation group(34 cases)by random number table method.The control group was treated with minimally invasive percutaneous plate fixation,and the observation group was treated with interlocking intramedullary nail fixation.The surgical indexes,fracture healing effect and complication rate at 18 months follow-up were compared between the two groups.Results The operation time and postoperative time of the observation group were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,and the length of the surgical incision was shorter than that of the control group,the differences were statistically significant(P<0.05).After 18 months of follow-up,the excellent and good rate of fracture healing in the observation group was higher than that in the control group,and the total complication rate was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion Compared with minimally invasive percutaneous plate fixation,interlocking intramedullary nail fixation in the treatment of lower tibial fractures has better curative effect,better surgical effect,faster recovery and fewer complications.
