摘　　要：目的探讨闭合复位髓内克氏针内固定治疗掌骨颈骨折的疗效。方法采用闭合复位髓内克氏针内固定治疗30例掌骨颈骨折患者。记录骨折复位、愈合情况,根据TAM功能评分标准评价临床疗效。结果26例骨折解剖复位,4例骨折功能复位。患者均获得随访,时间3~12个月。手部切口均一期愈合,未出现切口感染、皮肤坏死、骨折畸形愈合及骨不连等并发症。术后8~12周X线片显示掌骨骨折端对位对线良好,骨折端成角畸形纠正,骨折线消失。术后8~12周骨折愈合时根据TAM功能评分标准评价临床疗效:优24例,良4例,可2例,优良率为93.3%(28/30)。结论闭合复位髓内克氏针内固定治疗掌骨颈骨折,骨折复位理想,内固定可靠,操作简单,疗效满意。Objective To investigate the effect of closed reduction and intramedullary Kirschner wire internal fixation in the treatment of metacarpal neck fracture.Methods Thirty patients with metacarpal neck fractures were treated by closed reduction and intramedullary Kirschner wire internal fixation.The reduction and healing situation of the fracture were recorded,and the clinical effect was assessed with TAM function score criterion.Results Anatomical reduction was obtained in 26 cases and functional reduction in 4 cases.All patients were followed up for 3~12 months.All hand incisions healed in one stage,and there were no complications such as wound infection,skin necrosis,fracture malunion and bone nonunion.X-ray films at 8~12 weeks after operation showed good alignment of metacarpal fracture end,correction of fracture end angulation deformity,and disappearance of fracture line.At 8~12 weeks after operation,the clinical efficacy was evaluated according to TAM functional score criteria:excellent in 24 cases,good in 4,fair in 2,and the excellent-good rate was 93.3%(28/30).Conclusions Closed reduction and internal fixation with intramedullary Kirschner wire for metacarpal neck fracture have ideal fracture reduction,reliable internal fixation,and simple operation,with satisfactory effect.
